Who to contact?............................................................................17 1 Urgences / actions concertées --- Emergencies / Joint actions --- Emergencias /Acciones concertadas >>> JAPAN /日本 For almost five months, in Otsuchi, MdM Japan has been responding to the emergency caused by the earthquake. This action has been confirmed until the end of March 2012, and focuses on three main axes of intervention: - Primary healthcare in shelters and individual houses - Psychological consultations - Individual or group massages and relaxation therapy A team of dedicated specialists – psychiatrists, psychologists, nurses, acupuncturists, relief therapists – is the core of this work with the affected populations, under the supervision of the Iwate District health and social authorities. Since early May 2011, MdM Japan also intervenes in Otsuchi’s high school twice a week. Psychiatrists and psychologists from MdM receive students and staff in consultations, while therapists teach the students how to relax. MdM is currently taking care of about 530 people, who have been displaced due to the earthquake and live in 12 collective shelters. MdM is also in charge of providing moral and psychological support to those who have been relocated in prefab and individual homes. From April 4th to August 14th 2011, MdM Japan’s teams have carried out 835 medical and/or psychological consultations, 223 prescriptions and 797 relaxation sessions. >>> HAITI A specific Haiti newsletter All teams in the field have decided to produce a newsletter and to distribute it to partners (local and international NGOs, funders, local institutions…). The Responsables de missions, Presidents, Directors and desks of the MdM organisations concerned (Argentina, Belgium, Canada, France, Spain, Switzerland) all agreed and gave very positive comments. We are waiting for the 1st issue which will be in 3 languages: French, Haiti Creole and English. Cholera Après une période de stabilisation du nombre d’hospitalisations hebdomadaires à la fin avril, deux pics ont été observés, l’un à la mi-mai et l’autre pendant les deux premières semaines de juin. Le pic à la mi-mai était imputable aux nombres accrus de cas dans le Nord-ouest, l’Artibonite et l’Ouest (particulièrement Port-au-Prince) et le second en juin était imputable à l’Artibonite, au Centre et à l’Ouest, surtout la vaste flambée de cas à Port-auPrince. Cependant, bien que les rapports indiquent que maintenant le choléra a une tendance décroissante, dans certains départements on observe encore une augmentation des cas. Notamment dans le Nord-ouest, 1 208 cas suspects et 8 décès ont été rapportés par la Direction de la protection civile (DPC) entre le 21 et le 25 juillet. Dans le Département du Sud-est une augmentation de nouveaux cas de choléra a été signalée, en particulier dans la prison de Jacmel. Au 13 Juillet, environ 100 nouveaux cas suspects ont été rapportés sur un nombre de 360 détenus. Des mesures appropriées ont déjà été prises par les autorités sanitaires départementales pour traiter et isoler les malades hospitalisés et désinfecter les cellules et autres sites. >>> East Africa Food Crisis Kenya Two exploratory missions carried out by MdM France between late July and early August 2011 in the Dadaab area in Kenya allowed MdM to analyze the humanitarian and health needs as well as the local and international response deployed in response to the current food crisis. The Dadaab refugees’ camps area includes 3 camps hosting over 400,000 Somalis. However the drought and long-term conflict keep increasing the number of asylum seekers. The exploratory missions, one of which saw the participation of MdM Spain former president Dr. Ricardo Angora, allowed the identification of basic health needs and severe problems affecting the most vulnerable population of the district. Inadequate offer of services and health facilities and skilled human resources, shortage of water and sanitation conditions of the households and community areas, lack of medical and laboratory supplies and the interruption in the supply chain of essential drugs have been identified among the most urgent problems the public health system is challenged by in the area. The conditions in the Dadaab district hospital, the reference health facility for the refugee camps and the local resident community, are particularly worrying. Provided with very limited health staff and only one ambulance, the hospital has very limited surgical facilities for obstetrics emergencies which are referred to Garissa Provincial hospital, about 100 km away. 2 Following the results of the assessment activities, MdM proposes an intervention based on the improvement of the health services in Dadaab hospital and the primary care level of Garissa district through capacity building and support in terms of human and material resources. The action, planned to be launched at the end of August for 10 months, aims at delivering quality sexual and reproductive healthcare and child healthcare in emergency basic health structures, which would allow a better access to public health services, with a particular focus on the most vulnerable population. MdM will build additional structures and rehabilitate current buildings in Dadaab hospital, provide medical equipment, train and supervise medical staff on sexual and reproductive health, child health and nutrition and set up and implement medical protocols for clinical procedures. Moreover, the action will increase the quality of sexual and reproductive health and child health services in 10 Primary healthcare centers in the Garissa district by, inter alia: improving the referral system in the villages; providing medical equipment to the health centres; training and supervising medical staff on sexual and reproductive health, child health and nutrition; setting up protocols for clinical procedures; and developing health education curriculum on sexual and reproductive health, child health and nutrition. MdM intervention will be structured in cooperation with Kenya national health authorities and in partnership with WAHA International, a non-governmental organization specialised in the promotion of maternal and child health in rural and remote areas. photos: Pino Gonzalez Ethiopia Following MdM exploratory mission in the Dolo Ado refugees’ camp area in Southern Ethiopia, two camps – Bokolmanyo and Melkadida – have been selected for an MdM response. The response will aim at reducing the mortality and the morbidity among the Dolo Ado refugee and host populations via: the building four satellites clinics in Bokolmanyo and Melkadida camps and their equipment with basic medical material; the training of the medical staffs of the satellite clinics, the Ethiopian refugee agency (ARRA) clinics and the health centres in the two villages; training health educators on a health education curriculum; and organising health education sessions in Bokolmanyo and Melkadida camps and villages. MdM Spain and MdM France are in the process of jointly planning the intervention which is expected to be launched in the coming weeks. photo: Ewelina Gasiorowska Funding : MdM Belgium and MdM Germany have collected funds through their national fundraising platforms. Other MdMs might contribute in the coming weeks. The amounts collected (478KEUR as of August 26th) will be used by MdM France and MdM Spain for the implementation of their operations. Started with the Haiti crisis, this joint fundraising process is very successful and allows quick and easy support to the ongoing activities. Thanks to all the fundraisers who have contributed! 3 >>> Code de conduite En tant qu’organisation non-gouvernementale internationale Médecins du Monde se doit de veiller à la qualité de ses actions et pour ce faire d’améliorer ses pratiques d’action humanitaire. Cela implique de clairement définir les normes régissant la conduite des personnes s’engageant avec Médecins du Monde dans le cadre d’une mission sur le territoire d’origine d’une association ou dans un pays étranger. C’est la raison principale pour laquelle Médecins du Monde a décidé de définir des règles quant au comportement et à l’attitude des bénévoles, des volontaires, des adhérents et des salariés travaillant pour l’association. Le fameux "Code de Conduite", document adopté en 2006 par l'ensemble du réseau international de MDM vient de subir un petit "toilettage", rien sur le fond mais sur la forme avec pour changements principaux une mise en page en conformité avec la nouvelle charte graphique de MDM et le terme de "personnel local" remplacé par "personnel national". La DRI va prochainement revoir la version anglaise et espagnole avant de vous les transmettre à nouveau. Faites en bonne distribution ! Selon une enquête de 2009, l'ensemble des associations MDM distribuent ce document à leur personnel international. Cependant le flou persiste sur sa distribution et application pour le personnel national ainsi que les salariés des sièges. Minutes in French, English and Spanish are available on request: [email protected] >>> Common work and advocacy at a European level The DRI is coordinating the drafting of the future European strategy of the network, in relation with the national programmes. This strategy will include elements regarding operations, data collection, communication and advocacy. A first proposal was discussed at the Network Meeting of the Presidents and General Directors last June in Madrid. A new version will be shared and discussed with the network in the coming weeks. Some elements of this strategy: - strengthen the work with health professionals/organisations, - convey a strong message on medical coverage for all – including all migrants. - build a common strong message for the 7th of April, World Health Day in terms of communication. - strengthen our advocacy towards EU institutions on the right to healthcare for all, but also on their policy regarding infectious diseases. We plan to assess the Clinique du Parque in Sokodé city regarding: 1. PHC including reproductive, antenatal care and children's care 2. Assess the need to improve surgical capacity and provide eventually surgical training as a second priority. Ukraine: Cooperation MdM Germany - CAMZ We started cooperation with the local partner CAMZ (Medical Aid Committee in Zakarpattya) an NGO which works in Transcarpathia (Ukraine). The long-term goal of each project is the support, improvement and development of regional strategies on social and medical issues. Since 2009, CAMZ has worked on problems of refugees and asylum seekers in the Transcarpathian region. Burma: Eye Care project, Exploratory mission planned We are planning our first mission in Burma (Eye Care project) in December 2011. Currently the Memorandum of Understanding (MoU) has been finalised and handed over to the responsible Ministry in Burma. Bulgaria: Paediatric Training in Sliven for Roma project As a follow up to the international network workshop on Roma projects, the coordinator of the project in Munich will go to Sliven in order to evaluate the need for a training on children’s care which will be made later by a paediatrician from MDM Germany’s domestic mission in Munich. Many children seen in Munich are Bulgarian. The German foundation Renovabis is financing the project through MdM Germany, which is why MdM Germany’s advisor for international programs will monitor the project and report back to Renovabis. MdM France’s responsable de mission, Maragarita Salteri (also former desk officer for MdM Greece) will join MdM Germany for this trip at the end of August. Jubilee domestic missions in Munich & Stuttgart Since July 2009, the project MedMobil, a mobile clinic is offering basic medical care and prevention for vulnerable people, most of them living on the street, in Stuttgart. The occasion was used, not only to inform the press about the work done since 2009 but also to sincerely thank the project’s volunteer staff. Since September 2006, the project open.med works in Munich to grant people without medical coverage an access to basic medical care. The close cooperation with Café 104, a programme of the Bavarian refugee council, also allows open.med to provide legal advice to undocumented migrants. The increasing number of patients coming to open.med as well as the additional consultations - one for children and another for pregnant women - showed the need for access to health care for the Uninsured in Munich. open.med’s 5th jubilee is an occasion to organize a panel discussion on October 5th regarding access to care in Munich. This discussion includes speakers from various political parties as well as from the Migrants’ Medical Care project of the Order of Malta, also active in Munich. A benefit concert in the evening will also allow us to thank the whole volunteer team of open.med. MdM Germany: Funding for MdM’s project in Puntland The board of the German foundation Sternstunden approved partial funding of MDM France project in Somalia (200.000 Euro). >>> MdM Argentine / Argentina Perú: Misión de evaluación de situación de la salud colectiva en amazonía, Madre de Dios Un equipo llevó adelante una misión exploratoria con análisis de situación de salud en el Departamento de Madre de Dios, amazonía del Perú. Con entrevistas a referentes claves, asambleas comunitarias y visita a diversas zonas y comunidades indígenas de acceso por ríos. Se realizó un análisis sobre la epidemia de Dengue, las barreras de acceso a la salud intercultural, las rutas de trata de personas y explotación sexual de las mujeres y situación de salud de la minería artesanal contaminante. Médicos del Mundo contó con el apoyo de la Federación de Comunidades Nativas de Madre de Dios como de la Red de Promoción de la Mujer. Desde el 2007, Médicos del Mundo Argentina esta trabajando en la provincia de Chincha, en 4 comunidades rurales y de pescadores artesanales, departamento de Ica, en el Sur de Perú. 5 América Latina : Determinantes Sociales de la Salud: cómo enfrentar las inequidades en salud Debido a su trayectoria y abordaje metodológico, Médicos del Mundo Argentina fue convocada por la Organización Panamericana de la Salud (OPS) y la Organización de Estados Americanos (OEA) a la cumbre que se realizo los días 8 y 9 de agosto en San José de Costa Rica con motivo de la consulta regional a la sociedad civil, académicos y gobiernos sobre la próxima conferencia mundial de "Determinantes Sociales de la Salud" que se celebrará en Río de Janeiro, Brasil. Todos ellos debatieron y trabajaron sobre una propuesta regional latinoamericana de declaración y posicionamiento sobre los DSS. También, Gonzalo Basile (presidente), fue convocado por el Ministerio de Salud y Deportes de Bolivia y la Asociación Latinoamericana de Medicina Social (ALAMES) como uno de los 20 especialistas de América Latina convocados en la ciudad de La Paz a un taller-seminario para debatir sobre determinantes sociales de la salud. Provincia de Buenos Aires: Estrategia de Salud Ambiental El Equipo de Salud Ambiental de MdM lleva adelante un programa de promoción de ambientes saludables impulsando un estudio epidemiológico comunitario en salud ambiental buscando dar visibilidad a la relación entre la destrucción del ambiente, contaminantes, industrias sucias y su impacto directo sobre la salud colectiva de la población a nivel urbano. Este programa implica acciones de epidemiología comunitaria sobre barrios y asentamientos precarios urbanos teniendo en cuenta la profunda degradación de la cuenca hídrica del río Matanza-Riachuelo en la Provincia de Buenos Aires, principalmente en los distritos de Lomas de Zamora y La Matanza. Este trabajo se desarrolla junto a organizaciones vecinales como el Foro Hídrico y otras organizaciones sociales locales. Se busca que el trabajo de campo permita desarrollar propuestas que puedan traducirse en políticas públicas municipales y provinciales. También se desarrollará una publicación de la investigación, trabajo de campo y medición de contaminación (aire-suelo-agua) junto a la Universidad Nacional de Lanús y la Universidad Nacional de La Matanza, además de la formación de promotores barriales de Salud Ambiental, entre otras actividades. Haití: Trabajo en comunidades campesinas de Mirebalais y Lascahobas Ante el surgimiento del brote epidémico en el Departamento del Centro durante el 2010, Médicos del Mundo Argentina desplegó desde hace más de 6 meses en las comunidades campesinas de Flande y Gran Boukan (Mirebalais) y Juanpas y Roche Mulatre (Lacahobas) la Estrategia Promocional de Salud y Calidad de Vida enfocada en el combate al cólera en Haití. Se desarrolló un proceso de educación popular para la salud con conformación de grupos operativos de 230 Promotores de Salud y Organización Comunitaria que bajo la coordinación de las organizaciones de campesinos y mujeres junto con el Equipo de Gestión en Salud de MdM Argentina vienen trabajando la estrategia de construcción de territorios sociales libres de cólera y vigilancia de la salud colectiva con sus componentes de epidemiología comunitaria, promoción-protección de la salud y prevención. Así un total de 11 organizaciones haitianas campesinas y de mujeres locales desarrollan una intervención casa por casa en 3200 familias, con el desarrollo de línea de base epidemiológica comunitaria, con los componentes interculturales de medidas preventivas y promocionales frente al cólera en creole (cartilla y cartelera), complementando la etapa de cuidado de la salud mediante la identificación de determinantes sociales del cólera en cada hogar y la entrega de kits de cólera (cloro, jabón de toilet, jabón de ropa, SRO y aquatabs) y desarrollando medidas de bloqueo comunitario ante nuevos casos detectados buscando impactar sobre la incidencia. Esta experiencia fue tomada como ejemplo exitoso por el cluster local de la Organización Panamericana de la Salud (OPS) para replicarla en otras comunidades rurales del departamento del Centro. >>> MdM Belgique / Belgium / Belgica / Belgie: Mali : - Un projet urgence « équipes mobiles mixtes » va démarrer en août 2011. L’objectif de ce projet est d’aider les populations à faire face aux crises alimentaires et d’avoir une meilleure connaissance des déterminants de la malnutrition des enfants de moins de 5 ans des populations nomades pastorales de la région de Gao. Ce projet est réalisé en partenariat avec la Croix-Rouge française et Agronomes et Vétérinaires sans frontières. L’approche est donc mixte avec un volet santé humaine (préventif et curatif), un volet santé animale et un volet nutrition porté par la CRF. Une étude spécifique sera réalisée sur les populations nomades en particulier. - Les projets Soins de santé primaires des régions de Youwarou et Kidal suivent leur cours. Néanmoins, les conditions de sécurité dans la région de Kidal (et Gao) sont toujours extrêmement difficiles entraînant la rareté des missions de supervision des activités (grand banditisme). - Le projet de lutte contre le ver de Guinée à Kidal (dans le cadre du programme d’éradication du ver de Guinée – dracunculose) va redémarrer dans la région de Kidal. Les cas avérés de dracunculose ont toutefois été bien plus nombreux à Gao qu’à Kidal cette année mais le programme national montre une certaine résistance à accepter la proposition d’appui de MdM dans cette lutte sur la région de Gao. 6 - Un partenariat avec MdM Espagne est en cours pour l’enfance vulnérable, en particulier. Nous travaillons depuis 2007 à Bamako et sommes récemment associés à Keoogo, ONG locale partenaire au Burkina Faso. MdM Espagne lance un projet Enfance vulnérable à Saint-Louis. Tunisie : mission explo Mission exploratoire dans la région de Gafsa au cœur de la Tunisie, berceau de la révolution au printemps 2011. Un projet d’appui à la santé santé materno-infantile à travers un appui aux services de gynécologie des hôpitaux ainsi qu’un soutien à l’émergence d’une société civile active dans le domaine de la gouvernance du système de santé notamment est envisagé (amélioration de la collaboration et des synergies entre services). Une deuxième mission exploratoire devrait suivre en septembre afin de compléter et confirmer les hypothèses émises. Un financement sur la ligne Transition démocratique de la Coopération belge est envisagé. RDC : - Après une visite réalisée par la responsable médicale de MdM sur les aires de santé soutenues en RDC, nous souhaitons renforcer les réhabilitations de centres de santé et engager de l’énergie dans l’aménagement de sources pour garantir un accès à l’eau pour tous les centres de santé appuyés. - La rougeole connaît une énième épidémie en RDC et l’équipe de MDM-B a réalisé une campagne de vaccination sur 5 jours visant les enfants de 0 à 15 ans dans les zones de Katende et Tshikula. Les enfants sont vaccinés à plus de 90% en moyenne sur les deux zones mais 100% sont nécessaires pour endiguer la propagation de cette maladie, devenue endémique en RDC. Quelques familles congolaises sont en effet réfractaires à toute vaccination de leurs enfants, empêchant la cible de 100% d’être atteinte. Haïti : - Le choléra est encore létal en Haïti. Les équipes s’affairent toujours à contrer les sursauts de choléra qui touchent fréquemment Haïti. Cependant le nombre de nouveau cas est en baisse constante pour le moment. Dans ce contexte, MdM souhaite voir le gouvernement en formation prendre la mesure des dangers liés au choléra et doter ses structures de santé de plus de moyens tant financiers qu’humains. MdM-B vise un retrait dans le domaine du choléra pour fin novembre, estimant que le maintien du système d’alerte et des stocks de médicaments permettra une reprise instantanée des activités en cas de nouvelles flambées. - Une formation gynéco-obstétrique s’est tenue sur deux semaines dans le service materno-infantile de l’hôpital Chancerelle dans le département des Nippes en Haïti, avec la venue de 4 gynécologues belges pour renforcer les capacités des candidats spécialistes de cet hôpital. Evénements : - MDM-B a lancé Art Stickers, un nouveau concept de récolte de fonds mêlant art et solidarité. Art Stickers est un album type « Panini » à compléter par des autocollants, non pas de joueurs de foot, mais représentant des œuvres d’art belge contemporain. 15 artistes se sont investis dans le projet. Une première soirée d’échange de stickers a eu lieu et d’autres sont prévues dans les prochains mois. - Le Biketour Brussel-to-Paris, du 14 au 16 juillet, a mobilisé une équipe de 36 cyclistes avertis qui ont pédalé sur 440 km au profit de Médecins du Monde. Un challenge réussi avec un résultat net de 30 000 euros. >>> MdM Canada Haïti : - Intégration du traitement de la diarrhée aiguë en réponse au choléra à Cité Soleil : Le 5 juin 2011, mise en place d’une nouvelle section de traitement de la diarrhée aiguë à l’Unité de Traitement du Cholera (UTC) de CHAPI visant ainsi à évaluer et à traiter les différents types de diarrhées aiguës. Le Centre offre des services de laboratoire gratuits pour diagnostiquer le type de diarrhée, ce qui contribue à la sensibilisation et à une réponse rapide aux besoins spécifiques des patients. Les 15 postes de réhydratation orale (PRO) communautaires offrent un traitement pour les diarrhées aiguës, ainsi que pour le choléra. - Programme de vaccinations pour les enfants de Cité Soleil A Cité Soleil, MdM Canada intervient au niveau de la clinique semi-permanente du camp de déplacés Athlétique 2 et au niveau du Centre Sante du Fontaine (CSF). Dans le camp Athlétique 2, MdM Canada offre les services de consultations générales, consultations pédiatriques, consultations psychosociales, méthodes de planification familiale et suivi prénatal. Au Centre Santé du Fontaine, MdM Canada n’intervient qu’au niveau de la santé maternelle (Planification familiale et suivi prénatal). Les services offerts : planification familiale, consultation prénatale, examens de laboratoire et vaccination. De plus, et en partenariat avec le Bureau Communal de Cité Soleil, MdM Canada débute un programme de vaccination pour les enfants visant à prévenir les maladies évitables. L’objectif est de vacciner les enfants de moins de 5 ans qui viennent en consultation à la clinique fixe d’Athlétique 2 et CSF. 7 République Dominicaine : Choléra Depuis novembre 2010, 14 598 cas de Choléra ont été répertoriés en République Dominicaine. Depuis l’apparition du Choléra, MdM Canada a tenu des ateliers de sensibilisation à San Luis et dans les Bateys. Cette réponse efficace repose sur la capitalisation de l’expérience Haïtienne. En effet, l’équipe de MdM en République Dominicaine s’est déplacée à Port-aux-Prince afin d’y suivre une formation en février 2011. Depuis, aucun cas n’a été répertorié dans les Batteys et MdM participe à la vigilance épidémiologique dans tous les Batteys afin de coordonner ses actions avec les autorités de la santé publique. Montréal : Journées des coordonnateurs de Médecins du Monde Canada Du 1er au 5 août dernier, les coordonnateurs généraux se sont réunis au siège. En présence de Jean Saslawsky, de la Direction du réseau international, les coordonnateurs ont pris part à des ateliers sur les thèmes de la communication, du code de conduite pour les financements, de la préparation et réponse aux désastres, de la planification stratégique ainsi qu’un retour sur le bilan Haïti. Cet espace d’échange a permis de mieux définir les défis et perspectives tant sur le terrain, qu’au siège et en relation avec le réseau MdM. >>> MdM Espagne / Spain / España : Sierra Leona: sensibilización mediante teatro El 18 de junio 2011, MdM y YSCAT (Youth, Sport Culture and Tourism) organizaron en Kabala, Sierra Leona, un día de sensibilización mediante teatro sobre Salud Sexual y Reproductiva dentro del marco del proyecto: “Fortalecimiento de los servicios de salud sexual y reproductiva en el distrito de Koinadugu, desde una perspectiva de género y con el aumento de las capacidades de los hombres y mujeres del distrito para controlar su propia salud”. Durante varios meses un grupo de jóvenes de 10 escuelas segundarias del distrito ensayaron cada uno cuestiones relacionadas con Salud Sexual y Reproductiva (embarazos adolescentes, mortalidad materna e infantil, mutilación genital femenina, violencia de género, etc.) para presentar este día y delante de cada escuela. Cerca de 500 personas acudieron al evento donde los estudiantes presentaron obras de teatro de unos 30 minutos y donde el público resultó muy participativo y receptivo a los mensajes transmitidos. Mauritania: tuberculosis en prisión MdM ha diseñado un Plan de Intervención para proceder a la detección de más de un 20% de la población reclusa de la prisión de Dar Naim, Nuakchot, Mauritania. La prisión de Dar Nim con una capacidad para 350 reclusos en la actualidad esta superpoblada con una población reclusa de 1.250 personas viviendo en una situación de hacinamiento e insalubridad muy grande. En el último trimestre del 2010 aparecieron unos casos de tuberculosis y después de diversas reuniones con las autoridades sanitarias se permitió en junio del 2011 la entrada al recinto a MdM, que en colaboración con el CICR, la Fundación Noura, el jefe de la Prisión y las autoridades sanitarias se procedió a tomar las medidas necesarias para controlar y resolver la epidemia. Angola: nuevo proyecto En la provincia de Benguela (Angola) se está comenzando con el nuevo proyecto financiado por la Junta de Castilla La Mancha titulado Fortalecimiento del sistema de atención primaria de salud de la provincia de Benguela, con especial atención a la Salud Sexual y Reproductiva y al empoderamiento de la sociedad civil. El primer paso dado es el diseño de una óptima línea de base para medir los niveles de calidad de los resultados conseguidos durante todo el proyecto. Datos estadísticos, entrevistas y grupos de discusión son las principales herramientas cuantitativas y cualitativas utilizadas. España: manifiesto por el reglamento de los Centros de Internamiento de Extranjeros (CIE) La última modificación de la Ley de Extranjería, de diciembre de 2010, anunciaba la aprobación de un Reglamento de funcionamiento de los Centros de Internamiento de Extranjeros en el plazo de seis meses. A estas alturas, aún no se conoce borrador alguno, ni se ha abierto ningún espacio de diálogo entre las organizaciones sociales y el Gobierno con vistas a que el contenido del nuevo Reglamento asegure el máximo cumplimiento y protección de las personas internadas en los CIE. Por este motivo, diferentes organizaciones sociales han elaborado el Manifiesto Que el Derecho no se detenga a la puerta de los CIE, con el que se reclama una interlocución oficial que permita al movimiento asociativo y al resto de entidades implicadas, una participación real en el debate en torno al futuro Reglamento. MdM apoya el movimiento 15M (l@s indignad@s) Como parte de una ciudadanía que cree en la necesidad de un mundo más justo, MdM celebra las reivindicaciones de las miles de personas que han hecho suya la calle para reclamar un sistema que garantice la igualdad, la solidaridad, el bienestar humano, la equidad de género, la sostenibilidad ecológica y el derecho 8 a la educación, la salud, la participación política y al desarrollo pleno de todas las personas que habitan el planeta. Del mismo modo que la Coordinadora ONG para el desarrollo (CONGDE), de la que MdM forma parte, consideramos inadmisible que las medidas para luchar contra la crisis se lleven a cabo a costa de las personas más vulnerables y de sus derechos, perpetuando un sistema basado en valores alejados de la justicia y la solidaridad y en el que priman los mercados por encima de las personas y sus necesidades. >>> MdM France / Francia: République de Côte d’Ivoire En réponse à la crise postélectorale qui a affectée la Côte d’Ivoire en mars dernier, MdM France a ouvert une mission d’urgence le 20 avril 2011, dont la phase opérationnelle va prendre fin le 25 août. Elle a consisté à soutenir vingt et une structures sanitaires dans trois communes d’Abidjan où les populations y sont particulièrement vulnérables (Yopougon, Attecoubé, Adjamé/Williamsville), afin d’en permettre la réactivation et en assurer la fonction médico-sociale. Deux missions exploratoires ont aussi été conduites dans l’Ouest du pays dans deux régions particulièrement affectées par la crise et caractérisées par des situations critiques (rupture en médicaments extrêmement importante et généralisée; niveau d’équipement médical essentiel très faible et vétuste; niveau de formation du personnel médical faible; absence de réponse d’urgence pour le référencement des patients et leur prise en charge etc.). Une intervention de MdM France dans le Bas Sassandra a été retenue et est en cours d’élaboration. Elle permettrait d’apporter des capacités de réponse à l’urgence à un système sanitaire en danger, de remettre à niveau le système de référencement surtout en santé materno-infantile et de soulager fortement une région qui constitue le troisième plus gros bassin de population de la Côte d’Ivoire. Le projet proposé s’articule d’une part autour d’un volet de réactivation des services d’urgences obstétriques des 2 hôpitaux de référence de la zone (CHR Soubré et CHR San-Pédro), et d’autre part autour de la remise à niveau des services de maternité des centres de santé primaires principaux (13 centres soutenus) afin de répondre de façon intégrée aux besoins sanitaires des populations les plus vulnérables. Enfin, en priorisant la santé sexuelle et reproductive, le programme vise également les publics cibles du Ministère de la santé dans le cadre de la mise en place de sa politique de gratuité, à savoir les femmes enceintes et les enfants de moins cinq ans les groupes les plus vulnérables pour l’accès aux soins et les premières victimes de la désorganisation du système sanitaire. Au-delà des soins apportés aux populations ciblées, la perspective de partenariat avec MdM Suisse, qui assurera la suite du projet, participe de la motivation à prolonger notre présence. MdM demande une enquête sur le comportement de la police vis-à-vis des migrants dans la région de Calais Lettre de MdM adressée à Dominique BAUDIS, nouveau Défenseur Des Droits (DDD), demandant une enquête sur le comportement de la police vis-à-vis des migrants dans la région de Calais A l’occasion d’une saisine du Défenseur des Droits (DDD), le jour de sa nomination, par l’association No Border, soutenue par plusieurs associations, MdM a également saisi, s’inscrivant ainsi dans une démarche collective, le DDD de la situation faite aux migrants à Calais et plus particulièrement du comportement des services de police à leur encontre. Le Secours Catholique local a également adressé un courrier au DDD. Cette saisine a été accompagnée d’un rapport rédigé par la mission Nord-Littoral indiquant des faits constatés ou relatés par les migrants. Elle vise à demander qu’une enquête soit diligentée par le DDD afin de faire constater le comportement des services de police contraires à la déontologie. Le DDD a indiqué qu’il allait diligenter une enquête sur place. Fin de la pénalisation du séjour irrégulier : décision de la Cour de Justice de l’UE 28 avril / Communiqué ADDE, FASTI, GISTI, PRATIQUES, SAF, SMG, SM : La Cour de Justice de l’Union Européenne (CJUE) a rendu, ce 28 avril, un arrêt de portée considérable, qui marque un coup d’arrêt majeur aux politiques de pénalisation des étrangers en situation administrative irrégulière, en vigueur dans nombre d’États. 9 La Cour confirme qu’une législation prévoyant une peine d’emprisonnement pour le seul motif qu’un étranger se trouve présent de manière irrégulière sur le territoire malgré l’ordre qui lui a été donné de la quitter est contraire à la directive. The Court confirms that no detention sentence can be pronounced on the sole basis of irregular stay in a European country even if the foreigner has been officially requested to leave the territory. Dès aujourd’hui, les juges français, liés par cette décision de première importance, doivent « laisser inappliquée » toute disposition légale contraire à l’arrêt de la Cour, c’est-à-dire refuser de condamner à l’emprisonnement tout étranger qui ne s’est pas conformé à une décision administrative ou judiciaire d’éloignement. Droit au séjour des étrangers gravement malades : 80 militants anti-sida arrêtés aux portes du Sénat. AIDES et Act Up-Paris manifestaient contre la réforme du droit au séjour pour soins. Avec l’appui du gouvernement, le Parlement vient de voter un projet de loi qui modifie les termes de la loi précédente en remplaçant l’accès effectif aux soins par l’absence du traitement et en ouvrant la porte à la rupture du secret médical puisque c’est l’autorité administrative (et non pas les médecins) qui prennent la décision. L’article 17 ter ouvre la porte à l’expulsion de milliers d’étrangers malades vers des pays où l’accès aux soins est un luxe. Donc vers une mort prématurée. Rappelons que « malgré » une loi très favorable en France depuis près de 20 ans, les demandes de titres de séjour pour soins restent stables, et dans l’immense majorité des cas les demandeurs ont découvert leur pathologie plusieurs années après leur arrivée. The press kit includes data coming from a specific survey on access to vaccination among roma population in France, at the time where measles has become a public health problem in many European countries. A summary of 2 pages has been written by the DRI: please ask: A photo exhibition of Alain Keller was also presented: « Parias, les Roms en Europe ». >>> MdM Grèce / Greece / Grecia / Ελλάδα: Project in Patras & Igoumenitsa Medecins du Monde - Greece are witnessing with great concern the continuous use of violence against irregular migrants who live nearby the port of Patras under extremely bad conditions and which are trying to escape to Western Europe. Through a system of reporting that runs alongside our medical activities, first-hand testimonies have been gathered from this vulnerable population to whom we provide medical care. Migrants are being hit with batons on their limbs, genitals and heads, and these are only a few examples of this abusive “uniformed” physical brutality. This violence, which is being used without even the "excuse" of the resistance to an arrest, is an illegal and an inhumane abuse of power. In addition, ΜDM-Greece reports the exclusion of irregular migrants, potential refugees and people in need, from the services provided from the organisation’s mobile medical unit, an exclusion that violates the right for free medical care. Worth mentioning is the contradiction that these public servants systematically undermine actions paid by the Greek State. ΜDM-Greece has evidence proving all above statements and guarantees to act as the voice of people who are not able to be heard. Provision of primary health care and supply of medicines to people living in remote islands and villages in Greece. Two of MDM-Greece’s medical mobile units (dental and ophthalmological), staffed with volunteers (general doctors, pediatricians, ophthalmologists, dentists and gynecologists), visit isolated areas of Greece, such as remote villages and small islands. All persons that visited the clinic were examined by the doctors, but special emphasis was given to the elderly, which have poor access to the health care system. Additionally, the necessary medical examinations are done and finally medicines are provided free-of-charge to the one’s in need. >>> MdM Japon / Japan / Japón / 日本 : East Japan Emergency Mission MdM agreed to the request of local authorities to continue its psychological care mission at least until the end of March 2012. Activities will be focused more on people living in temporary individual houses, 2000 of which should have been built by the end of July. An election is planned end of August to choose the new mayor of the city Otsuchi where MdM is intervening, following the death of the previous mayor during the tsunami. 10 Tokyo Project Since the beginning of the year, the activities of Tokyo Project for homeless people with mental disabilities continued despite the participation of Dr Suimei Morikawa, psychiatrist in charge of Tokyo Project, to the East Japan Emergency Mission. The number of homeless people followed by MdM team amounts now to 60. Operation Sourire in Madagascar A new mission will be organized in August following the request by Dr Francois Foussadier. From Japan, Dr.Yoza and Ms.Harada are expected to join in. Operation Sourire in Bangladesh A team of 6-Japanese and 2-French people operated in Dakka (Bangladesh) from February 24 to March 3, 2011. 2nd Global Forum on Human Resources for Health MdM Japan participated to the 2nd Global Forum on Human Resources for Health held in Bangkok January 24-29. The forum, co-organized by the Global Health Workforce Alliance, JICA, WHO and Prince Mahidol Award Conference, welcomed around 900 participants from Ministries of Health, Foundations, NGO, the UN, scholars, etc. World Social Forum 2011 in Dakar MdM Japan participated to the World Social Forum in Dakar (Senegal) together with Japanese and African NGOs (see precedent What’s Up Doc for a report on Dakar’s forum (editor’s note). A Night for the Benefactors of Doctors of the World Japan 2011 The charity event held on July 1st at the Residence of France attracted more than 260 guests, and raised more 7.5 million yen. 30 French cuisine chefs offered their best dishes, while volunteer musicians, masters of ceremonies, flower arrangement masters, and sponsors were also contributed to the evening’s success. >>> MdM Pays-Bas / Netherlands / Paises Bajos / Nederland Indonesia The mother and child health program in Aceh province has started up again with a new general coordinator with midwifery background. The aim is to decrease maternity and newborn mortality rates. The program will focus on training of midwives and community development program in all 28 villages of Permata subdistrict in cooperation with the District Health Office and partners. Burma MdM-NL has undertaken an assignment to MdM-France in Pyapon (delta zone: district previously affected by the Nargis cyclone) which has led to a project proposal for continuation of the mother and child health program. When financing is granted by an institutional donor, MdM-NL will take over this MdM-France project. National Program for Undocumented Migrants For continuation of the national program in the Netherlands for the next 4 years, proposal development for the National Postcode Lottery donor for 1st September has been undertaken by a consultant. Under this financing proposal we intend to: a) roll out the current point of services to 3 other cities in the Netherlands for mediation, referrals and prevention programs b) do research on the effects of detention on health in detention centres where increasingly more undocumented migrants are being held. Our aim is to increase the services for them in and outside of detention centres. c) roll out the service for registering refusals of health care for undocumented migrants and providing supporting information to other intermediaries to improve mediation for these migrants. Elderly care Project options for other vulnerable groups such as elderly with multi-morbidity are being looked into together with an elderly care professional in terms of issues with access to health care health-care and the chances for financing. First findings indicate vulnerabilities within a group of elderly who still live at home and for which services are not always available. Possibilities for MdM-NL to work with volunteers to improve the access to health-care have been identified. 11 >>> MdM Portugal World Water Day Celebrations in Guinea Bissau On 22 March was celebrated World Water Day. As in previous years, the PIASHEB (project in partnership between EDF and MdM, funded by the European Commission and the IPAD) celebrated this day in GuineaBissau. This year the main activity of celebration was held at the school Prabir. The animators of the project were present, together with the school board, the governor of Prabir (community leader) and management representatives. The activity began with dance competitions for children. Later, the technicians IEC had a play that mocks the bad behaviour of capture, transport and water use. At the end of the play was given the opportunity for children to reflect on the satire, while encouraging school doubts clarified through an awareness session on water. Throughout the activity were distributed sweaters, photo-novels, calendars and pamphlets to raise awareness in the participants. The whole team has assessed positively the event, not only for the organization and dynamics, but also because the children have revealed a wealth of knowledge about the issues of water, sanitation and hygiene ... fruit of PIASHEB! East Timor: Campaign of Measles Immunization MdM participates in the National Campaign of Measles Immunization, in East Timor. On June 13th has begun the campaign of the National Immunization Measles, a campaign that came in response to an outbreak of measles in the country, which began in the districts of Dili and Ermera. Mozambique: We have entered a new phase of the project “Estamos Juntos na Ilha” (“We’re Together in the Island”), at Mozambique. We are training local groups to enable them, in an integrated manner, to be active in preventing HIV/AIDS and Malaria. 12 New magazine for MdM Portugal After 29 editions, we decided to reformulate the MdM Magazine – “FACE”: multi-thematic, new layout (with more pictures, less text), more dynamic and appealing. The 1st issue was in May 2011. Its contents will always be developed on our website. This is a continuous and integrated action, begun in 2010. In parallel, we are developing a new official website, with new dynamic, categories, appealing and intuitive when it comes to navigation, according to the guide lines of the international network. The website will be launched in September. Music bands join for the 8 goals of the Millenium Eight Portuguese bands attended the challenge and decided to give voice to the objectives, which are not mine, that is not theirs, which are all of us. These are the eight Millennium Development Goals set by the United Nations. For eight weeks these bands give the voice for one of the objectives and rely on everybody to join this cause. This initiative is undertaken by the Portuguese NGDO’s Platform under the ContractProgram funded by the Portuguese Institute for Development Support. The MdM Portuguese delegation is one of the partners of this event which implies a lot of media promotion and dissemination. >>> MdM Royaume-Uni / United Kingdom / Reino Unido Submission to the Committee on the Elimination of Racial Discrimination In June Doctors of the World UK made a submission to CERD (The Committee on the Elimination of Racial Discrimination, http://www2.ohchr.org/english/bodies/cerd/) regarding health inequalities based on our findings at Project:London. We highlighted the impact that proposed NHS reforms will have on the homeless, refugees and asylum seekers. The key threats being the weak system for challenging discrimination and the impact of increased commercialisation. New regulations in UK on access to healthcare for foreign nationals On the 1st August the UK government implemented changes to access to healthcare for foreign nationals which will affect service users at Project:London. It is not completely positive but there are some significant gains including: - Exemptions for refused asylum seekers on Section 4 and Section 95 support from charging secondary care - Recognition that diagnosis should not be refused if the medical issue is urgent 13 - Exemptions for unaccompanied minors Some of the bad news from the review includes: - Sharing data about debts over £1000 for NHS treatment with UKBA (United Kingdom Borders Agency) so that they can refuse people who request extended entry to the UK, citizenship or to return to the UK - A potential review in 2012 on extending charges for primary care; reassessing the criteria for ordinary residence; and the introduction of compulsory health insurance for visitors to the UK DoW UK obtains quality recognition DOW UK is please to have received PQASSO Quality Mark at Level 2. On May 31st 2011 the leader of the enquiry that has been going on since April 2010, regarding health care for undocumented migrants and asylum seekers released the report. It was handed over to the minister of Social Affairs, Göran Hägglund. In conclusion, the investigator Erna Zelmin, noted that Sweden today does not live up to the international obligations that we have signed regarding the right of all to optimal health. She also believes that current law has an unacceptable degree of discretion in the extent of care to asylum seekers and undocumented migrants. Therefore, she proposes that undocumented migrants and asylum seekers should be entitled to benefits under the same conditions as residents and registered in Sweden. The investigator has also attached special importance to children's needs. Therefore, she also suggests that age should not limit the right to health care because children's access to care is influenced by parents' rights. Médecins du Monde Sweden welcomes the proposals for a legislative change, ie. that care for undocumented migrants and asylum seekers should be given according to needs. Médecins du Monde Sweden now calls on the Government to take on the enquiry’s result and write a new law to ensure that Sweden does not continue to exclude people from health care. This means that there should be no discrimination on the patients, regardless of their administrative status. Médecins du Monde Sweden wants the government to know that the enquiry was limited from the beginning, since it excluded certain other groups who today also do not have the right to health care in Sweden. For example, EU citizens and third country nationals, who have residence in another EU country, who lack social security. These groups are only entitled to emergency medical care to full payment. "I have hopes for the new law but would still like to point out that every week we get more and more patients to Médecins du Monde’s clinic in Stockholm and that there are groups other than those who have been assessed as still being excluded from care. We received funds from Forum Syd www.forumsyd.org (a democracy and rights organization and a platform for NGO’s in Sweden) for an information project. MdM Sweden will publish theme magazines and do lectures for health care staff and students on the subject and show how the MdM networks work to reduce mother mortality. The first magazine came out. >>> MdM Suisse / Switzerland / Suiza: Haïti : Depuis fin mai, Haïti doit faire face à une nouvelle flambée de l’épidémie de choléra qui est dans une phase endémique. Même si le centre de traitement de choléra (CTC) a fermé, les autorités locales ont interpelé MdMSuisse afin qu’elle apporte son soutien technique et financier et ce sont maintenant des structures fixes qui accueillent les gens. 14 Bénin : Après l’ouverture d’un premier centre de prise en charge de la drépanocytose à Abomey en Novembre 2010, MdM-Suisse définit les conditions d’implantation d’une autre antenne dans la ville de Parakou. Pour compléter ses activités, MdM-Suisse en partenariat avec la MUFELD (Mains unies des familles engagées dans la lutte contre la drépanocytose) et le CPMI-NFED (centre de prise en charge intégrée du nourrisson et de la femme enceinte atteints de drépanocytose), a collaboré à la parution d’un guide de bonnes pratiques pour les parents concernant la drépanocytose. Palestine : Un nouveau projet en collaboration avec le Palestinian Medical Relief Society (PMRS) et la Centrale sanitaire suisse romande (CSSR) démarre le 1er septembre. Cette collaboration vient en soutien au centre de référence en pédopsychiatrie d’Hébron. Il a pour but de réaliser une série de supports audio-visuels destinés à la prévention et à la déstigmatisation de la santé mentale et de la santé en général dans les Territoires occupés palestiniens. Cameroun : En 2010, l’ONG neuchâteloise REA Cameroun œuvrant pour l’accès à l’éducation, sollicitait l’aide de Médecins du Monde-Suisse pour étendre ses activités. L’idée étant de créer un bloc sanitaire afin de mettre l’accent sur la prévention du paludisme mais aussi de mettre en place un programme de prise en charge des maladies de l’enfance afin de diminuer la mortalité infanto-juvénile de la commune de Koupa Kagnam au Cameroun. Au cours du mois de janvier 2011, Médecins du Monde-Suisse a donc lancé une mission exploratoire sur le terrain qui a démontré qu’il y avait non seulement un besoin mais également une volonté de la part des autorités publiques de la zone concernée de mettre sur pied une intervention MdM. C’est pourquoi MdM a accepté de participer au projet sous réserve de pouvoir mobiliser les ressources financières nécessaire. Lausanne : Permanences Blanches : Ce programme d'écoute et de prévention concernant les questions de santé auprès des travailleurs et des travailleuses du sexe à Lausanne se poursuit avec une permanence nocturne le mercredi de 22h à 2h en antenne mobile et une permanence diurne le jeudi de 14 à 16 heures. 3 infirmières de MdM Suisse travaillent dans ce projet mené en partenariat avec l’association Fleur de Pavé. Une visite de terrain de Nathalie Simonnot sur le bus a engendré beaucoup d’échanges et des propositions seront formulées prochainement pour faciliter le travail et améliorer la qualité du service rendu. Chaux de Fonds : Réseau Santé Migration : MdM Suisse et l’EPER (association jusqu’à aujourd’hui partenaire dans le projet) entament une réévaluation du projet puisque cette dernière ne désire pas poursuivre son activité dans le projet. Un groupe de travail va donc commencer à redéfinir le projet. La santé n’est pas un luxe Du 19 au 24 juillet, Médecins du Monde-Suisse a exposé certains des éléments de l’exposition « la santé n’est pas un luxe » au Paléo Festival de Nyon. Lors de cet événement musical qui est le plus grand festival open air de Suisse, Médecins du Monde-Suisse s’est encore une fois approprié tous les codes du luxe en utilisant des vitrines blindées pour présenter des médicaments, des stéthoscopes, des vaccins et des « cartes d’assurance gold » pour affirmer que la santé ne devrait pas être une luxe. Adoption : formation le 3 novembre Le jeudi 3 Novembre 2011, une journée de formation organisée par MdM Suisse et Espace Adoption ayant pour thème « l’adoption et la santé de l’enfant adopté : le rôle du pédiatre » aura lieu. Livres à lire / Books to read: Médecins Sans Frontières: La Biographie (Anne Vallaeys) démystifie les origines du mouvement “French Doctors”, de l’avant Biafra au schisme après le débat sur un bateau pour le Vietnam qui a mené à la création de MdM. Le livre décrit et analyse les actions de MSF jusqu’au début des années 2000. Ce texte examine les tensions humaines et décisions éthiques derrière les moments charnière de MSF.
