Fort de son expérience sur la concrétisation de partenariats public/privé, le pôle de compétitivité Eurobiomed s’est investi dans l’innovation pour les maladies rares depuis sa création. RARE 2011 est une opportunité unique pour les acteurs du pôle non seulement de s’informer sur les enjeux de la recherche pour les maladies rares, mais aussi de porter leur voix sur les sujets clefs qui permettront de proposer de nouveaux traitements et diagnostics pour ces pathologies. Stimuler la recherche dans ce domaine, éviter les idées reçues, montrer que les maladies rares ont ouvert la voie pour d’autres domaines clefs en santé comme la médecine personnalisée, sont autant de sujets et d’échanges que le pôle souhaite provoquer lors de ces 3 jours. Car RARE 2011 n’est pas qu’un événement où se réunissent science et stratégie, c’est une dynamique pour que les entreprises du pôle aillent de l’avant vers de nouveaux développements. Grâce à l’ensemble des partenaires, nos financeurs et tous les membres du comité, RARE 2011 s’annonce riche en échanges, auxquels j’aurai plaisir à participer. www.rare2011.com T o advance innovation and partnerships for the benefit of patients – that is the objective of RARE 2011, the 2nd edition of the Eurobiomed meetings on rare diseases, held in Montpellier. The Eurobiomed competitive cluster boasts solid experience of producing tangible results from public-private partnerships and it has been investing in innovation for rare diseases ever since its creation. RARE 2011 is a unique opportunity for stakeholders from the cluster not only to gather more information on the stakes of research on rare diseases, but also to express their views on key issues, helping to pave the way for the development of new treatments and diagnostics for these pathologies. In the course of these 3 days of discussions, the cluster hopes to help stimulate research in this field, dispel received ideas and demonstrate that research on rare diseases has blazed trails for other key health issue, such as personalised medicine. RARE 2011 is not only an event where science and strategy come together, it is also dynamic, aimed at driving the cluster’s companies to work ever closer to bring new developments nearer. Thanks to its array of partners, funders and all the committee members, RARE 2011 promises a wealth of successful exchanges, and I am delighted to be able to take part in them. Jacquie BERTHE President of Eurobiomed Président d’Eurobiomed S ’il est une belle cause de santé publique, en 2011, c’est bien celle des maladies rares. Que de besoins médicaux non satisfaits, que d’attente de la part des patients, que de science disponible ou en cours de développement, qui est ou sera à la disposition des développeurs de solutions ! Les entreprises, traditionnellement peu enclines à s’intéresser à ces pathologies affectant chacune peu de malades, participent maintenant activement au développement de thérapies personnalisées pour les maladies rares, contribuant ainsi à la mise au point de diagnostics et de traitements pour des effectifs limités. De nouveaux modèles économiques doivent prévaloir avec, au-delà des chercheurs et des entreprises, la participation des investisseurs, des financeurs et celle des décideurs institutionnels qui doivent fixer les cadres incitatifs tout en protégeant les malades. Dans ce contexte, l’implication des associations de patients est essentielle d’un bout à l’autre de la chaine. Mais il faut que toutes les « parties prenantes » se rencontrent et travaillent ensemble pour que les choses changent ! car chaque partenaire, isolément, ne peut rien. C’est bien là l’ambition de RARE 2011, l’édition 2011 des Rencontres Eurobiomed sur les Maladies Rares, centrée sur « l’innovation et les partenariats au service des malades ». « RARE », une formule courte et incisive, traduit la volonté d’aller droit au but et d’éviter les détours qui font perdre des ressources et du temps. Après le succès de la première édition de ces rencontres, fin 2009, la gouvernance du pôle Eurobiomed a décidé d’inscrire « RARE » dans la durée et de l’organiser tous les 2 ans. Nous avons aussi voulu aller plus loin dans l’interaction, dans la facilitation des rencontres, dans la dynamique et dans la recherche de solutions. Car nous sommes intéressés par les solutions aux problèmes plus que par les problèmes eux-mêmes… Deux ateliers « recherche de solutions » en témoignent. Nous espérons que les propositions seront riches et que les travaux qui en seront issus serviront de base à l’édition 2013 de RARE. En souhaitant le meilleur succès à ces Rencontres, nous tenons à remercier toutes celles et ceux qui se sont mobilisés pour contribuer à la préparation de cet événement et à son déroulement. Nous remercions sincèrement les sponsors privés et les financeurs publics qui ont apporté un soutien aussi enthousiaste qu’indispensable. I f there is one great public health cause in 2011, it has got to be rare diseases. So many unsatisfied medical needs, so much expectation from patients and so much science available - or under development and soon to be available - to help the developers of solutions! In the past, companies have been reluctant to show an interest in these pathologies that only have a small number of patients, but they are starting to understand that this is an attractive sector and that developing it complements personalised therapeutic medicine, which also entails developing diagnostics and treatments for limited numbers of people. It is time to pursue new economic models that harness the participation not only of companies and researchers but also, of course, of the funders as well as the public authorities, who must lay down incentivising frameworks and protect patients. Obviously, it is also essential that patient associations are involved from the beginning to the end of the processes. But all of the “stakeholders” must meet and work together so that things change! Because none of the partners can achieve anything by itself. This is the real ambition of RARE 2011, which is the 2011 edition of the Eurobiomed Meeting on Rare Diseases. The overriding theme is “Innovation and partnerships in the service of the sick”. ‘RARE’ is a simple and incisive abbreviation that reflects the eagerness to go straight to the target and avoid all the detours that waste time and resources. After the success of the first edition in late 2009, the governors of the Eurobiomed cluster decided to make this a regular occurrence and stage a similar event every 2 years. We were also motivated by a desire to push interaction even further, facilitate even more meetings, advance the dynamic and deepen research for solutions. We are more interested in the solution to problems than in the problems themselves. The fact that there will be two “researching solutions” workshops reflects this vision. We hope that many interesting proposals will be put forward and that they will give rise to significant work between now and the 2013 edition of RARE. We hope that these meetings will prove tremendously successful and we would like to express our gratitude to all those who helped with the preparation and the smooth running of it. We would also like to thank the private sponsors and the public funders whose enthusiastic and crucial support is greatly appreciated. La mobilisation des associations de patients, les compétences des chercheurs, l’implication des médecins et l’engagement de certains industriels permettent chaque jour des avancées importantes afin de redonner un espoir aux patients qui attendent un traitement. Le 1er Plan Maladies Rares a formalisé la nécessité d’améliorer les connaissances des pathologies rares, de dynamiser la recherche et de structurer la prise en charge des malades. Le 2ème Plan permettra de poursuivre et parfaire le travail commencé. Nous pouvons être fiers de ce qui a été accompli mais cela ne suffit pas. En dépit des avancées de la science, des pistes thérapeutiques prometteuses et de l’implication forte d’acteurs dévoués, le nombre de médicaments orphelins mis sur le marché reste encore trop faible. Mobilisons-nous car nous tenons entre nos mains les solutions tant attendues par ceux qui souffrent. Ces nouvelles Rencontres Eurobiomed sur les Maladies Rares relèvent ce défi et apportent un souffle nouveau dans le monde des maladies rares. Les organisateurs ont compris que pour aller plus loin et plus vite, les partenariats publics-privés sont clés, et que ce sont les actions et les solutions qui comptent, au-delà des discours convenus. Réunir tous les partenaires, qu’ils soient chercheurs, industriels, financiers ou associations de patients pour permettre « les rencontres » qui vont dynamiser la recherche et faire foisonner les programmes en cours, est un projet ambitieux mais totalement réalisable. C’est bien là la valeur ajoutée des pôles de compétitivité, dont Eurobiomed est un fleuron, créés il y a quelques années par le gouvernement français et qui sont devenus le fer de lance d’un nouveau dynamisme entreprenarial dans notre pays. L a recherche dans le domaine des maladies rares a connu des avancées majeures au cours des dix dernières années. Aujourd’hui, prenant appui sur les formidables développements tant dans le domaine de l’exploration des génomes que du développement des thérapeutiques, les acteurs de la recherche mènent une réflexion en profondeur afin que leurs approches prennent en compte ce qu’il est tenu de considérer comme une révolution de la démarche scientifique, des outils technologiques et du cadre éthique. Malgré des progrès significatifs, il reste tant à faire. Découvrir les causes et les mécanismes de nombreuses maladies rares, créer des modèles exploratoires, identifier des molécules thérapeutiques pertinentes, et surtout accélérer le transfert des études pré-cliniques vers les essais cliniques. Sans aucun doute, seule la convergence des efforts permettra d’avancer de manière rapide et significative; c’est l’objectif que les fondateurs de RARE se sont fixé. Initié et fondé par Eurobiomed, RARE est un modèle unique de colloque centré sur les maladies rares. Ses fondateurs et organisateurs ont résolument, dès la première édition, souhaité associer les chercheurs universitaires, fondamentalistes et cliniciens, aux partenaires industriels pharmaceutiques et biotechnologiques, aux organismes de réglementation dans le domaine de la santé, aux décideurs institutionnels, et bien-entendu aux malades via les associations qui les représentent. Deux mille onze, année de naissance du 2nd plan national maladies rares et de création de la «Fondation maladies rares», dont la philosophie est de favoriser la mise en relation des acteurs de la recherche et du soin ainsi que le développement de partenariats académiques-privés, ne pouvait donc être mieux choisie pour la deuxième édition de RARE. L’aboutissement des actions d’Eurobiomed dépend de la volonté de chacun de nous. Nous devons montrer l’exemple, initier les partenariats et donner de la visibilité à nos actions. Au vu des résultats positifs, d’autres suivront. Nous pouvons faire de ces rencontres une réussite nationale et maintenant déjà européenne. En 2013, la composante européenne et internationale devrait être encore plus marquée. Nous serons portés par l’espoir de millions de patients qui attendent aujourd’hui un traitement pour demain. En favorisant la collaboration entre les différents acteurs,RARE 2011 permettra de partager l’état des connaissances et d’explorer les enjeux et besoins à travers de multiples expériences et perspectives. Faciliter l’accélération du développement de nouvelles thérapeutiques, améliorer l’accès à celles existantes ou à venir en vue de l’amélioration de la qualité de vie et de traitement des malades, telle est la mission que RARE 2011 s’est fixée, et c’est la raison pour laquelle nous nous sommes tous engagés aux côtés de ses organisateurs. I O n the heart of Europe, France is recognised as a front-runner on rare diseases. The mobilisation of patient associations, the skills of the researchers, the involvement of doctors and the commitment of some industrialists make it possible to achieve significant advances every day and give hope to patients awaiting the discovery of a treatment. The 1st Rare Diseases Plan formally expressed the need to improve knowledge of rare pathologies, give extra impetus to research and structure the treatment of patients. The 2nd plan seeks to pursue and perfect the work that has begun. We can be proud of what has already been achieved but it is not enough. In spite of the scientific progress, the emergence of promising therapeutic trails and the ardent involvement of dedicated stakeholders, the number of orphan drugs available on the market is still too low. Let us rally to action, because the solutions that so many suffering people are waiting for are in our hands. These new Eurobiomed meetings on Rare Diseases strive to rise to this challenge and serve as a new catalyst for action on rare diseases. The organisers have understood that to go farther, faster, public-private partnerships are essential and actions and solutions are what count, beyond discourses. Bringing together all the partners - whether they are researchers, industrialists, funders, or patient associations – for meetings that energise research and enrich the programmes in progress, is an ambitious but eminently attainable goal. That is the added value generated by the clusters of which Eurobiomed is a shining example. Created a few years ago by the French government, it has become a driving force of a new entrepreneurial dynamism in our country. What fruit Eurobiomed’s action will reap depends on the level of determination that each of us has. We have to give an example, form partnerships and give visibility to our actions. If we show positive results, others will emulate us. We can make these meetings a national triumph, indeed an European one. In 2013, the European and international component should be even more significant. We will be carried by the hope of millions of patients who are waiting today for a treatment tomorrow. ver the last ten years, major advances have been made in research on rare diseases. Today, by drawing on the brilliant developments in genomic exploration and therapies, research actors are conducting an indepth review to ensure that their approaches factor in what can be rightfully considered as revolutionary progress in terms of the scientific process, technological tools and ethical framework. In spite of these great strides, there is still a way to go. We still have to discover the causes and mechanisms of many diseases, build exploratory models, identify the right therapeutic molecules and, above all, speed up the transfer of pre-clinical studies to clinical trials. There is no doubt that fast and meaningful advances can only be made by a convergence of efforts. And this is precisely what RARE aims to achieve. Launched and founded by Eurobiomed, RARE is a unique model of a symposium focused on rare diseases. It has been the conviction of the founders and organisers from day one that it should bring academic researchers, basic researchers and clinicians together with partners from the pharmaceutical and biotechnology industries as well as health regulatory bodies, institutional policy-makers and, of course, patients via the associations that represent them. No more appropriate year could have been chosen than 2011 to host the second edition of RARE, as this is when the 2nd national rare diseases plan was initiated. This same year was established the ‘Rare Diseases Foundation’, whose philosophy entails forging ties between those involved in research and developing private-academic partnerships. By fostering collaboration between the various actors, RARE 2011 will allow for the pooling of knowledge and the exploration of issues and needs through an array of experiences and outlooks. The mission that RARE 2011 has set itself is to facilitate faster development of new therapies, increase access to existing and future ones so as to improve the quality of life and treatment of those living with the diseases. That is the reason why we are all committed to supporting the efforts of the organisers. COMPOSITION QUALITATIVE ET QUANTITATIVE : Un flacon contient 400 unités* de vélaglucérase alfa. Après reconstitution, un ml de solution contient 100 unités de vélaglucérase alfa. *Une unité enzymatique est définie par la quantité d’enzyme nécessaire à la conversion d’une micromole de p-nitrophényl β-D-glucopyranoside en p-nitrophénol en une minute à 37ºC. DONNEES CLINIQUES (*) : Indications thérapeutiques : Traitement enzymatique substitutif à long terme indiqué chez les patients atteints de la maladie de Gaucher de type 1. Posologie et mode d’administration : Administration sous la surveillance d’un médecin expérimenté dans la prise en charge de la maladie de Gaucher. Une administration à domicile supervisée par un professionnel de santé peut être envisagée uniquement chez les patients ayant reçu au moins trois perfusions et tolérant bien leurs perfusions. Posologie : Posologie recommandée : 60 unités/ kg une semaine sur deux. Ajustements posologiques possibles au cas par cas en fonction des objectifs thérapeutiques atteints et de leur évolution. Mode d’administration : Voie intraveineuse exclusivement. À administrer au cours d’une perfusion IV de 60 minutes, avec usage d’un filtre de 0,22 μm. Contreindications : Réaction allergique sévère à la substance active ou à l’un des excipients. Mises en garde spéciales et précautions d’emploi : Hypersensibilité : Réactions d’hypersensibilité rapportées chez des patients au cours des études cliniques. Prévoir le matériel médical approprié lors de l’administration de la vélaglucérase alfa. En cas de réaction sévère, suivre les standards des mesures médicales du traitement d’urgence. Envisager le traitement avec une attention particulière chez les patients ayant présenté des symptômes d’hypersensibilité à un autre type d’enzymothérapie substitutive. Réactions liées à la perfusion : Réactions indésirables les plus fréquemment observées dans les études cliniques, pour la plupart d’intensité légère. Prise en charge en fonction de la sévérité de la réaction : réduction du débit de perfusion, traitement médicamenteux tel que des antihistaminiques, antipyrétiques et/ou corticostéroïdes, et/ou interruption puis reprise de la perfusion avec une durée de perfusion plus longue. Traitement préalable par antihistaminiques et/ou corticostéroïdes est susceptible d’empêcher l’apparition de réactions ultérieures. Immunogénicité : En cas de réaction liée à la perfusion sévère ou d’absence (2) d’effet ou de perte d’effet chez un patient, tester la présence d’anticorps et informer la société des résultats obtenus. Fécondité, grossesse et allaitement : Femmes en âge d’avoir des enfants : Période d’exacerbation de l’activité de la maladie pendant la grossesse et la puerpéralité possible. Bien peser les bénéfices et les risques du traitement. Surveillance étroite de la grossesse et des manifestations cliniques de la maladie de Gaucher pour une adaptation thérapeutique individuelle. La majorité de ceux-ci concerne des produits pour des cancers rares. Le développement clinique d’un médicament orphelin doit répondre aux mêmes exigences que celles de tout autre traitement en démontrant un rapport bénéfice / risque positif. Il existe un certain nombre de barrières au développement des médicaments orphelins qui sont liées essentiellement à un manque de compréhension des mécanismes physio-pathologiques, à un manque de soutien public apporté aux premières étapes du développement clinique et à un trop faible engagement de l’industrie pharmaceutique. A l’heure actuelle, ce développement est d’un coût élevé et il existe en moyenne 7 ans entre la désignation de Médicament Orphelin et l’Autorisation de Mise sur le Marché. Le taux d’attrition est supérieur aux autres produits de santé et est plus élevé quand le sponsor est de petite taille et la prévalence de la maladie inférieure à 1 sur 10 000. Il est donc important d’envisager la mise en place d’études épidémiologiques, de développer des partenariats privé-public et de faciliter les autorisations par des AMM sous circonstances exceptionnelles avec un engagement de l’industriel pour la mise en place d’études complémentaires. Depuis 2005, des Plans de Gestion des Risques sont demandés pour tout dossier d’AMM, pour un médicament avec une nouvelle substance active ou pour des changements significatifs, tels qu’une nouvelle voie d’administration d’un médicament existant ou enfin pour un médicament avec un risque important identifié. Ces Plans de Gestion des Risques permettent de confirmer la tolérance et le bon usage du médicament. Ces études sont difficiles car doivent être mises en place dans 27 pays et très souvent concernent une population cible très large (patients traités et non traités). D’autres études peuvent aussi être demandées au niveau national par la Commission de Transparence et/ou le Comité économique du médicament. 20 The stakes of pre-and-postmarket authorization development and clinical assesment Annick Schwebig, President General Manager – Actelion Pharmaceuticals France Since the implementation of the European regulation on orphan drugs, 62 products have been authorised in Europe for the treatment of 82 rare diseases. The majority of these relate to rare cancers. The clinical development of an orphan drug must comply with the same criteria as those for any other treatment by demonstrating a positive risk/benefit ratio. There are a number of barriers to the development of orphan drugs that are primarily linked to a lack of understanding of the physical and pathological mechanisms, a lack of public support during the initial stages of clinical development and the low level of involvement of the pharmaceutical industry. As things stand, this development is expensive and at least seven years elapse between the designation of an Orphan Drug and it receiving Authorisation to Market. The attrition rate is greater than for other health products and is even greater still when the sponsor is small and the incidence of the disease is less than 1 on 10,000. Therefore, it is important to carry out epidemiological studies, develop private-public partnerships and facilitate authorisation to market in exceptional circumstances and involve industrial operators for the production of additional studies. Since 2005, it has been compulsory to compile Risk Management Plans to accompany any application for Authorisation to Market a drug with a new active substance or featuring significant changes such as a new way of administering an existing medicine, or for any drug that carries a significant identified risk. This Risk Management Plan formally sets out the tolerance levels and right usage guidelines of the product. These studies are difficult because they must be conducted in 27 countries and very often concern a very large target population (treated and untreated patients). Ségolène Aymé, Directrice de recherche émérite, INSERM SC11 / Orphanet A mixed assessment can be made concerning the findings of research in rare diseases. On one hand, there have been spectacular scientific advances in the dissection of the biological mechanisms of these diseases, leading to an ever-growing array of diagnostic tools. On the other hand, therapeutic research, despite undergoing significant development thanks to the incentives put in place, is only devoted to a small number of diseases. If we examine what has created this situation, we find that there is a certain number of resources that are key for R&D and these must be shared with the academic and industrial worlds and the community of patients and their entourage. We are talking, for the most part, about data bases, both clinical data repositories from healthcare systems and data collected specifically for research. Agreement must now be reached on the shared governance of these data bases, bearing in mind the regulatory considerations for industrial producers and agencies, scientific competition between academic researchers and the ethical considerations for patients. Standards must be devised for the nomenclature of diseases, their clinical description and the inter-operability of the data bases. Le bilan des retombées de la recherche dans le domaine des maladies rares est contrasté. D’un coté les avancées de la science sont spectaculaires dans la dissection des mécanismes biologiques de ces maladies, conduisant à une offre sans cesse élargie en termes d’outils diagnostiques. D’un autre coté la recherche thérapeutique, bien qu’en plein développement grâce aux incitations mises en place, ne concerne qu’un petit nombre de maladies. L’analyse des déterminants de cette situation montre qu’un certain nombre de ressources sont clés pour la R&D et doivent être partagées entre le monde académique, le monde industriel et la communauté des malades et de leur entourage. Il s’agit des bases de données en général, qu’il s’agisse d’entrepôts de données cliniques issues des systèmes de santé ou de données collectées spécifiquement pour la recherche. Il faut maintenant se mettre d’accord sur la gouvernance partagée de ces bases de données, en raison des enjeux réglementaires pour les industriels et les agences, des enjeux de compétition scientifique pour les chercheurs académiques et des enjeux éthiques pour les malades. Il faut développer des standards pour la nomenclature des maladies, pour leur description clinique, et pour l’interopérabilité des bases de données. Ségolène Aymé, Senior research director, INSERM SC11 / Orphanet 21 Résumés / Abstracts Conférence / Conference N°3 Les enjeux de l’évaluation médico économique, le bénéfice pour le patient Dr François Meyer, Conseiller du Président, Haute Autorité de Santé (HAS) En Europe, la prise en charge par l’assurance maladie des médicaments, des dispositifs et actes médicaux relève de chaque Etat membre qui décide du financement et de l’organisation de son système de santé. Chaque Etat est libre de constituer son « panier de biens et services remboursables » selon ses propres critères et priorités et de le financer selon les modalités qu’il juge appropriées. Les décisions relatives à la constitution et à la gestion dynamique de ce panier de biens et services sont éclairées par des évaluations scientifiques, selon des critères définis par les réglementations nationales. En France c’est la Haute Autorité de santé (HAS) qui a la mission d’effectuer ces évaluations scientifiques. Elles peuvent avoir pour objet le bénéfice clinique pour les patients concernés (service médical rendu, amélioration du service médical rendu). Il s’agit d’évaluer, sur la base des données disponibles, le gain en santé qui peut être apporté par le médicament. Cette évaluation est toujours difficile, compte tenu du caractère limité des données disponibles pour un nouveau médicament. Cette difficulté est plus marquée pour les médicaments orphelins, pour lesquels on dispose de moins de données, tant sur la maladie (quelle histoire naturelle ?) que sur le traitement : données limitées en termes de durée d’exposition au traitement, de nombre de patients étudiés et portant souvent sur des critères de substitution et non des critères cliniques. La HAS doit également évaluer le service rendu à la collectivité, en évaluant différentes dimensions non cliniques, dont la dimension économique. Ce point est essentiel pour répondre à la préoccupation d’un bon usage des ressources collectives affectées à la santé, tant en termes d’efficience que d’égalité d’accès à des soins de qualité. 22 The stakes of medicoeconomic assessment, benefit for the patient Dr François Meyer, Director of the Health Technology Assessment Division at HAS. In Europe, the extent to which medicines and medical provisions and procedures are covered by national health insurance depends on the individual member state, with each one deciding on the funding and arrangement of its healthcare system. Every state is free to compile the range of publicly paid-for range goods and services according to its own criteria and priorities and to fund it using the methods that it deems appropriate. The decisions relating to the compilation and dynamic management of this range of goods and services are informed by scientific evaluations that comply with criteria set out in national regulations. In France, these evaluations are conducted by the National Authority for Health (Haute Autorité de santé - HAS). The evaluations may seek to establish the clinical benefit for the patients concerned (i.e. medical service provided or improvement of the medical service provided) and they entail assessing, on the basis of available data, the health effects of a medicine. The evaluation is always difficult because of the necessarily limited amount of data available on new medicines. This difficulty is particularly pronounced for orphan drugs, for which there is less data available both on the treatment and on the diseases (what is their natural history?) owing to the short duration of exposure to treatment, the number of patients studied and the fact that data is often focused on substitution criteria rather than clinical criteria. The HAS is also responsible for evaluating the service provided to the governing body, by assessing various non-clinical aspects, including the economic aspect. This is an essential consideration when it comes to ensuring that the resources that the government assigns to health are properly used, both in terms of efficiency and of equality of access to healthcare. www.rare2011.com Conférence / Conference N° 4 Développement de synergies et partenariats pour les maladies rares : le modèle de la «Fondation maladies rares» Nicolas Lévy1,2 1GIS-Institut des maladies rares – Plateforme maladies rares, Hôpital Broussais, Paris ; 2Inserm UMR_S 910 et Département de Génétique Médicale, Campus hospitalier et Universitaire de la Timone, Marseille. Malgré la dynamique qui s’est créée en France autour de la recherche sur les maladies rares ces dernières années, il existe encore une grande hétérogénéité de l’état des recherches selon les maladies rares. La recherche souffre toujours de la fragmentation des approches et du manque de coordination quant aux sources et à l’utilisation des données. Ces faiblesses tiennent essentiellement à la dispersion des structures dédiées aux maladies rares, en particulier de celles dédiées à l’aide à la recherche, chacune ayant des objectifs propres, souvent convergents mais sans pour autant qu’une stratégie commune soit à ce jour clairement établie. Une plus grande réactivité vis-à-vis des nouvelles approches technologiques ainsi que des moyens importants sont à prévoir afin de mener les études indispensables et de faire face aux défis qui sont à relever. Les points faibles identifiés, tout comme la rareté de chacune des maladies considérée comme distincte, sont des facteurs qui, jusqu’à peu, ont parfois freiné l’implication des industriels du médicament pour le développement de thérapeutiques spécifiques. En effet, si un grand nombre de maladies rares bénéficie dorénavant d’outils diagnostiques performants, trop peu d’entre elles bénéficie de traitements voire d’essais thérapeutiques dédiés. Dans le paysage actuel, le renouvellement du Plan National Maladies Rares (PNMR2) pour la période 2011 à 2015 tout comme l’intérêt grandissant des industriels du médicament et des biotechnologies pour les maladies rares représentent une évolution réellement très favorable aux malades. Dans ce contexte, la réflexion engagée par le GIS-institut des maladies rares, avec ses partenaires associatifs et institutionnels, a ainsi conduit à la création d’une Fondation maladies rares afin que les malades puissent bénéficier des retombées de la recherche, et maintenir la France dans le rang scientifique de tout premier plan au niveau international. Cette «Fondation maladies rares» se veut être une structure de réflexion stratégique et de financement de projets à travers des programmes de recherche et la mise en relation des acteurs de la recherche et du soin. Elle a pour objectifs de mettre à disposition des acteurs du domaine, un «continuum» d’expertises indispensables au développement de la recherche sur les maladies rares, intégrant la recherche fondamentale et clinique, la méthodologie sur les collections de données cliniques et biologiques, le soutien au montage des phases précoces d’essais thérapeutiques et les aspects de coopération internationale. Etroitement liée aux missions de soin et de prise en charge des malades, la «Fondation maladies rares» s’est fixée comme missions de fédérer les compétences en recherche, faciliter l’accès aux plateformes technologiques, et de créer des synergies favorisant l’émergence de thérapeutiques spécifiques. Ces objectifs ne sauraient être atteints sans le développement des partenariats avec les acteurs industriels qui, déjà engagés aux côtés des malades, bénéficieront des retombées des programmes de partenariats initiés. Les missions de la «Fondation maladies rares» s’inscrivent dans la cohérence et la complémentarité avec les axes définis dans le cadre du plan national maladies rares PNMR2 dont elle est également une des mesures phare. La Fondation, grâce à ses partenariats publics, associatifs et privés, participera activement au maintien du domaine «maladies rares» à un niveau de priorité indispensable à la prise en charge des malades et à une gestion globale des problématiques de recherche. Notwithstanding the dynamic that has been created in France in recent years on rare diseases, there are still wide disparities in terms of research. Research still suffers from a fragmentation of approaches and a dearth of coordination relating to the sources and use of data. These weaknesses are generally down to the fact that the bodies dealing with rare diseases remain disparate, particularly when it comes to research, with each one pursuing its own objectives; and even though these, in many cases, overlap with others, there is still no clearly defined common strategy. Greater responsiveness to new technological processes and significant tools should be sought in order to carry out vital studies and surmount the challenges that exist. These shortcomings represent factors that, along with the rarity of diseases considered as distinct, until recently discouraged medicine companies from developing specific therapies. While there are now effective diagnostic tools for a large number of rare diseases, there are still too few specific treatments, or even trials, for them. The current context, with the renewal of the National Rare Diseases Plan (PNMR2) for the period 20012015, and the growing interest of medicine and biotechnology companies in rare diseases, reflects a change that is genuinely positive for patients. In the light of this, the discussion undertaken by the GIS-Institute of Rare Diseases with its associative and institutional partners, lead to the creation of the Rare Diseases Foundation. The goal of this is to ensure that patients enjoy the benefits of research and to maintain France in the international vanguard of scientific advances. The Foundation intends to serve as a body that stimulates the strategic development and funding of projects through research programmes and the networking of research and healthcare operators. It strives to provide actors in this area with a “continuum” of indispensable expertise on the development of research on rare diseases, drawing on basic and clinical research, methodological know-how for clinical and biological data collection, planning early phases of therapeutic trials and offering support with the various aspects of international cooperation. Directly linked to care and treatment activities for patients, the Rare Diseases Foundation aims to unify research, facilitate access to technological centres and generate synergies that beget specific therapies. These goals can only be attained through the formation of partnerships with industrial actors who will benefit from the results of the partnership programmes formed by dint of working with the patients. The missions of the Rare Diseases Foundation complement those set out in the National Rare Diseases Plan (PNMR2) and, indeed, it constitutes one of the flagship measures of that plan. The Foundation, thanks to associative and private public partnerships, will play an active role in keeping rare diseases a high enough priority to ensure patients are looked after and research issues are managed in a holistic way. 23 Résumés / Abstracts Conférence / Conference N°5 La révolution génomique du diagnostic des maladies rares Jean-Louis Mandel, Université de Strasbourg / IGBMC La révolution actuelle des méthodologies de séquençage d’ADN permet l’identification accélérée de gènes responsables lorsque ‘ils sont mutés, de maladies génétiques. Il est également très important de transférer ces technologies puissantes pour le diagnostic moléculaire. Ce diagnostic seul permet de connaître l’étiologie exacte d’une telle maladie. Ce diagnostic est très difficile, voir impossible en pratique, par les techniques classiques de séquençage, lorsque le nombre de gènes potentiellement mutés est important, pour une pathologie donnée: cardiomyopathies hypertrophiques (> 10 gènes), syndrome Bardet-Biedl (16 gènes connus), rétinites pigmentaires ou surdités congénitales (>> 30 gènes pour chacune de ces pathologies), et enfin probablement plus de 200 gènes pour le retard mental L’utilisation du séquençage à haut débit va permettre des progrès considérables, qui restent à mettre en œuvre, mais les questions d’interprétation des variations de séquence vont se multiplier. Le passage au diagnostic en milieu hospitalier demandera des efforts de financement, et pose le problème en période d’accréditation d’une technologie rapidement évolutive. Il faudra se poser la question des applications possibles au diagnostic préconceptionnel généralisé pour de nombreuses maladies monogéniques sévères ou au diagnostic prénatal non invasif, notamment de la trisomie 21: y a-t-il des dérives eugénistes à craindre, ou faut il au contraire permettre de telles applications? 24 Genomical revolution of rare disease diagnosis Jean-Louis Mandel, Strasbourg University / IGBMC The current revolution in DNA sequencing methodologies makes possible the rapid identification of genes whose mutation causes genetic disorders. It is very important that this powerful technology is also transferred to be used on molecular diagnosis. This is the only diagnosis that can confirm the exact etiology of such a disorder. It is needed in clinical research to ascertain the specific natural history of these disorders and is vital if appropriate medical care is to be provided. It also informs the genetic counselling of families and, for severe diseases, can enable prenatal or pre-implantation diagnosis. It is very difficult – even impossible, in practice - to make this diagnosis with traditional sequencing techniques when the number of potentially mutated genes is high for a given disorder: hypertrophic cardiomyopathies (> 10 genes), Bardet-Biedl syndrome (16 known genes), retinitis pigmentosa or congenital auditory imperceptions (> 30 genes for each of these disorders), and, finally, probably more than 200 genes for mental retardation. The use of high-speed sequencing is going to allow for significant progress, which remains to be implemented, but so too will there be an increased number of questions of interpretation. Introducing this diagnostic method into hospitals will require financial efforts and raises the problem of the accreditation period of a technology that is evolving rapidly. La découverte d’une molécule thérapeutique guérissant le malade est certes l’espoir que nous attendons tous, mais d’autres approches thérapeutiques existent, elles améliorent souvent la qualité de vie des patients et ne doivent pour cela pas être méconnues. Ces approches thérapeutiques existent dans des domaines extrêmement variés. Au travers de quelques exemples très divers, nous voulons montrer que la recherche pour les maladies rares concerne des entreprises qui n’ont probablement aucune idée de l’apport quelles peuvent avoir dans la prise en charge des maladies rares. Les dystonies génétiques sévères en particulier secondaires à des mutations du gène Dyst1 ont vu leur prise en charge révolutionnée par l’implantation stéréotaxique d’électrodes de stimulation cérébrale profonde. Les enfants atteints du syndrome de Fanceschetti ont parfois une agénésie des oreilles associée à une surdité profonde. Du fait de l’absence de pavillon, il n’est pas possible de proposer une réhabilitation auditive classique. La mise au point de prothèses auditives à conduction osseuse ou de prothèses implantables permet à ces enfants de corriger leur déficit auditif et d’acquérir un langage adapté. Les patients atteints de syndrome d’Elhers Danlos présentent des troubles articulaires, vasculaires et cutanées avec une grande fragilité cutanée. La mise au point de vêtements de compression en tissu élastique à base de viscose, d’élasthane et de polyester leur assure une protection et une prévention des traumatismes cutanés. Plus de 3 000 maladies rares ont pour conséquence des troubles cognitifs de l’adaptation et des comportements chez l’enfant. Chez les enfants porteurs de délétion 22q11 (1/4000 naissances), des programmes informatiques de remédiations permettent d’enrichir leurs compétences socio-émotionnelles. Ce logiciel, né de recherches en neurosciences cognitives, améliore le raisonnement, la reconnaissance des émotions et éventuellement lhumeur (anxiété et dépression). De manière générale, l’ordinateur est un outil précieux au service du savoir. Il permet, par le biais de supports pédagogiques et de rééducations ludiques, une plus grande autonomie de l’enfant face à divers troubles des apprentissages. Ces exemples sont représentatifs de l’immense champ des thérapeutiques non médicamenteuses qu’il importe d’explorer dans la prise en charge des patients atteints de maladies rares. We usually think that therapeutic research for rare diseases only relates to medicines. It is certainly the case that the discovery of a therapeutic molecule that can cure patients is the hope that drives us all but it is also true that other therapies exist and they often enhance the quality of life of patients. Therefore, they should not be neglected. These therapeutic approaches exist in a wide variety of fields. We aim to illustrate, via a selection of diverse examples, that research on rare diseases concerns companies that probably have no idea of the contribution that they could make to the treatment of rare diseases. The handling of severe genetic dystonia, in particular subsequent mutations of the Dyst1 gene, has been revolutionised by the implanting of stereotactic electrodes for deep cerebral stimulation. Children with Fanceschetti syndrome sometimes have ear agenesis and profound deafness. Owing to the absence of a pinna, the standard method for restoring hearing is not an option. However, the development of bone conduction hearing aids enables these children to correct their hearing deficit and learn an adapted language. Patients with Elhers Danlos syndrome present articular, vascular and cutaneous problems with high cutaneous fragility. The development of elastic tissue compression clothing made from viscose, spandex and polyester provides them with protection and prevents cutaneous traumas. More than 3,000 rare diseases cause children cognitive deficiencies with adaptation and behaviour. For children with 22q11 deletion syndrome (1 in 4,000 births), remedial computer programmes can increase their social and emotional skills. This software, which was designed for cognitive neuroscience, improves reasoning, recognition of emotions and, ultimately, moods (treating anxiety and depression). Generally speaking, computers are precious tools for advancing knowledge. Computerised educational tools and fun rehabilitation supports can give children greater independence in coping with various learning difficulties. These examples are representative of the immense field of nonmedicine based therapies that can be explored to treat patients with rare diseases. 25 Résumés / Abstracts Conférence / Conference N°7 Maladies rares: laboratoire d’innovation pour la santé humaine Serge Braun, Directeur Scientifique, AFM (Association Française contre les Myopathies) Si les maladies rares trop longtemps négligées sont reconnues aujourd’hui comme un véritable problème de santé publique tant en France qu’en Europe, les efforts de recherche à l’échelle mondiale sont très proportionnellement aux maladies fréquentes beaucoup plus faibles. Pourtant, l’identification de gènes responsables de maladies rares a permis de comprendre des mécanismes biologiques fondamentaux de la division et la survie cellulaire. Si les maladies rares sont particulièrement complexes à aborder d’un point de vue thérapeutique, elles sont pourtant à l’origine de nombreuses pistes particulièrement innovantes : thérapie génique « chirurgie » du gène, saut d’exon, translecture des mutations STOP. Ces nouvelles stratégies thérapeutiques, validées dans les maladies rares, ouvrent des voies nouvelles pour des maladies fréquentes. Ainsi le saut d’exon qui consiste de manière très spécifique et contrôlée à forcer la fabrication par les cellules de protéines tronquées peut conduire à la synthèse de protéines fonctionnelles ou inactives. L’inactivation de protéines de tumeurs ou de virus permet ainsi d’entrevoir de nouveaux anti-cancéreux ou de nouveau anti-viraux. Les thérapies mises en œuvre pour l’adrénoleucodystrophie, la beta-thalassémie, les immunodéficiences, les épidermolyses bulleuses, les cécités génétiques, ouvrent des pistes thérapeutiques pour des maladies fréquentes du cerveau, la drépanocytose, le SIDA, les ulcères cutanés diabétiques ou la dégénérescence maculaire liée à l’âge. La Progeria (maladie qui touche 25 familles en Europe) qui ouvrent la voie au vieillissement général et à des essais dans le cancer et le SIDA . C’est ainsi que s’est construite la stratégie d’intérêt générale de l’AFM. Créer les conditions de l’émergence de preuves de concept clinique sur un certain nombre de maladies dites « modèles » pour ensuite permettre la généralisation des thérapeutiques et faire effet de levier industriel et financier. 26 Rare diseases : innovation laboratories for human health Serge Braun, Scientific director, AFM (French Association against Myopathies) Rare diseases that have been neglected for too long are now acknowledged to be a real public health problem in France and Europe, yet global research efforts into them remain very low when compared in proportional terms with research into common diseases. However, identifying genes responsible for some rare diseases has made it possible to understand the basic biological mechanisms of cell division and survival. Even if rare diseases are especially difficult to approach from a therapeutic viewpoint, they nevertheless have given rise to numerous particularly innovative avenues of exploration such as “surgical” gene therapy, exon jumping and read through of STOP mutations. These new therapeutic strategies, which were validated in the course of researching rare diseases, open up new opportunities for dealing with common diseases. Accordingly, exon jumping, which consists of forcing, in a very specific way, cells to produce truncated protein, can lead to synthesis of functional or inactive protein. The ability to render the protein of a tumour or virus inactive paves the way towards new ways of tackling cancer and viruses. Also, the therapies deployed for adrenoleukodystrophy, beta thalassemia, immunodeficiencies, epidermolyses bullosa and genetic blindness open up possibilities for treating common brain diseases, sickle-cell anaemia, AIDS, cutaneous diabetic ulcers, or macular degeneration linked to age. Similarly, progeria (a disease that affects 25 families in Europe) offers possibilities for combatting ageing in general and trials for treating cancer and AIDS. This is how AFM formed its strategy of widespread interest. L’année suivante un groupe de généticien proposait une initiative similaire pour la région de la méditerranée (Nat Genet, 2010 ; 42(8)) : un centre d’excellence consacré à l’analyse des génomes des populations méditerranéennes et du Proche- et MoyenOrient quelles que soient leurs nationalités, leurs appartenances ethniques et religieuses. Cette initiative revêt une importance capitale en raison de la consanguinité élevée qui prévaut dans ces populations et de la situation géographique de ces Pays lieux de passage obligé et carrefours de nombreuses migrations, pacifiques ou non, depuis la sortie de l’Homme de l’Afrique. En effet, la consanguinité, dépassant les 30% de mariages entre cousins germains dans les zones rurales, est responsable d’une fréquence considérablement accrue des maladies génétiques autosomiques récessives très rares ou quasi absentes dans les populations où la consanguinité n’existe plus. L’identification des gènes encore inconnus impliqués dans ces maladies, suivie immédiatement de la compréhension de la physiopathologie correspondante, permet d’accéder à la connaissance de maillons insoupçonnés, et indécelables ailleurs, là où il n’y a plus de consanguinité, de la Biologie Cellulaire et de la Physiologie normale et pathologique des systèmes biologiques concernés. L’intérêt de tous ces résultats est inestimable pour l’ensemble de la Communauté scientifique et médicale mondiale et, bien sûr, pour les patients qui en attendent beaucoup pour l’amélioration de leurs traitements, tout en permettant le conseil génétique et le diagnostic anténatal dans les familles à risque. In Cairo in June 2009, the president of the United States, Barack Obama, spoke of the importance of international collaboration for scientists and suggested creating centres of scientific excellence to resolve global problems. The following year, a group of geneticists made a similar proposal for the Mediterranean region (Nat Genet, 2010; 42(8)): a centre of excellence devoted to the analysis of the genomes of the populations of the Mediterranean and the Near and Middle East, irrespective of their nationalities, ethnicity or religious identity. This initiative is vitally important because of the high rate of consanguinity that prevails among these populations and the geographic location of these countries, which have been crossroads for many migrations, peaceful and otherwise, ever since mankind spread out from Africa. Consanguinity, which exceeds 30% of marriages between first cousins in rural areas, is responsible for a significant incidence of recessive autosomal genetic diseases that are very rare or virtually absent from populations where consanguinity no longer exists. The identification of the still unknown genes that cause these diseases – which would enable understanding of the corresponding physiopathology – can drive the discovery of hitherto unsuspected, and elsewhere undetectable, links where there is no longer any consanguinity, in Cell Biology and normal physiology and pathophysiology of the biological system concerned. The value of all these findings would be incalculable for the whole global scientific and medical community and, of course, for patients who await improvements in their treatments. It would also enable genetic counselling and antenatal diagnosis of families at risk. Most rare diseases are absent in ICD10 and those with a specific code are often misclassified. As a consequence, morbidity and mortality due to genetic diseases is invisible in health information systems. To overcome this difficulty, Orphanet (www.orpha.net) has established a partnership with WHO to ensure a fair representation of rare diseases in general. The Orphanet nomenclature of rare diseases is a stable one, directly exploitable by information systems and freely accessible at orphadata.org. If adopted, it will provide the opportunity to trace RD in the multiple sources of medical data, an opportunity to better know the incidence and prevalence of individual diseases, but also a source of data for clinical research. This data is of high relevance in the R&D process as many initial steps are now done in silico. Clinical databases are more and more valued both by Industry, and by Academic research groups if they are able to cross talk with other biological databases. They help better understand the pathogenesis of the diseases, generate hypothesis for drug target, and provide insight on the proximity of diseases which could benefit from progresses developed for other diseases. www.rare2011.com SESSION 2 : SESSION 3 : Partnering to optimize the use of patient data to improve clinical research and healthcare Improving access to expertise and quality of care Patient registries and databases constitute key instruments for the development of clinical research in the field of rare diseases (RD), and the improvement of patient care and healthcare planning as well as social, economical and quality of life outcomes. They are the appropriate way to pool scarce data without bias for epidemiological and/or clinical research. They are vital to assess the feasibility of clinical trials, to facilitate the planning of appropriate clinical trials and to support the enrolment of patients and to assess the impact of new interventions. Registries of patients treated with orphan drugs are particularly relevant as they allow the gathering of evidence on the effectiveness of the treatment and on its possible side effects. Databases are expensive to establish and maintain and require the cooperation of many healthcare providers. They are of high interest to researchers, industrial partners, healthcare professionals, patients, and, ultimately, for the community. It is difficult to separate research conducted by the non-profit and for-profit sectors, as researchers from both sectors are often involved in the same projects. Whilst this enables effective technology transfer, it also gives rise to concerns about conflicts of interest. There is a need to promote confidence in research based on data collections to foster the establishment of quality data repositories, to ease and speed up clinical research in the field of RD and OD and to provide data to regulatory bodies, to bodies in charge of assessing the clinical added value of orphan drugs and to reimbursement bodies. Centres of Expertise and European Reference Networks in the field of rare diseases (RD) are mentioned in the Communication from the Commission “Europe’s challenges” (11.11.08) and the Council Recommendation on an action in the field of rare diseases (08.06.09), as well as in the recommendations for National Plans and Strategies for Rare Diseases and in Articles 12 and 13 of the Cross-Border Healthcare Directive. An analysis of the current situation in Europe will be presented. With regard to centres of expertise, there are four categories of countries in Europe: those which have a policy regarding RD and have established centres of expertise within this framework; those which have established centres of expertise, though not specifically for RD; those which have started the process of designating centres of expertise for rare disease or will soon; and those which have no centres with this denomination, although they have centres with all the characteristics of a centre of expertise. With the Cross Border Healthcare Directive, all the European countries will have to designate their centres of expertise. Serge Braun, PharmD, PhD, is Scientific Director of AFM, the French Musculal dystrophy Association. He has 10 years experience in the University and 10 years in the biotechnology sector. He obtained his degree in Pharmacy and a Ph.D in Pharmacology from the Louis Pasteur University of Strasbourg, France. He joined from 1990-1992 as postdoctoral fellow, the Neuromuscular Center, School of Medicine of the University of Southern California in Los Angeles, USA. From 1992 to 1995, he returned as research assistant to the School of Pharmacy of Strasbourg working on the pathogenesis and treatment of different genetic and acquired neuromuscular diseases. In 1995, he joined Transgene (Strasbourg, France), the largest French Biotech company, where he became Scientific Director and developed his career in the field of gene therapy of genetic diseases and of immunotherapy of cancer. Several products have now reached the stage of phase II/III clinical trials. Cet Etablissement a notamment pour mission de répondre à des besoins thérapeutiques qui ne peuvent être comblés par les médicaments de l’industrie pharmaceutique – traitement de certaines maladies rares, ou formes galéniques spécifiquement adaptées au nouveau-né ou à l’enfant et dans certains cas aussi à la personne âgée (dosage unitaire, forme pharmaceutique) ; il dispose pour cela de la capacité réglementaire (selon la définition du Code de la Santé Publique) pour développer, produire, contrôler et commercialiser ces médicaments qualifiés d’« indispensables». Elle travaille en ce moment pour obtenir une AMM pour trois de ces médicaments qui ont actuellement le statut de ‘préparation hospitalière’. A côté de cette activité ciblée ‘médicaments orphelins’, Marie-Caroline HUSSON est impliquée dans différentes associations professionnelles et sociétés savantes de pharmacie clinique, l’European Society of Clinical Pharmacy notamment dans laquelle elle représente la France au Conseil d’administration. A specific mission he has is to meet therapeutic needs that can not be met by the drugs developed by pharmaceutical companies - treatment of few rare diseases, or formulations specifically adapted to the infant or child and in some cases also to the elderly (unit dose, dosage form), ; to do that he has the regulatory capacity (as defined in the Code of Public Health) to develop, produce, manage and market the drugs qualified as «essential.» She is currently working for a marketing authorization for three of these drugs that currently have the status of ‘ hospital preparation ‘. In addition to this ‘orphan drugs’ activity, Marie-Caroline Husson is involved in various professional associations and societies of Clinical Pharmacy, the European Society of Clinical Pharmacy particularly in which she represents France at the General Committee. He was appointed as member of several state commissions within the Ministry of Health, namely the “Transparency Commission” for evaluation of pharmaceutical products (198992), the National Health Account Commission (1992-1995), the task force for guidelines in economic evaluation of medical technologies, the Committee for the accreditation of physicians networks (1997-2000), the Commission for national tarifff of medical procedures (2004). He is non executive member of the board of the CNAMTS, the main public health insurance fund in France (2003), and expert to the Parliamentary Office for Health Policy Evaluation (2003). He also acts as expert for the European Union and was rapporteur of a working party preparing the round-tables on the Completion of the Single Market for Pharmaceuticals (1996-1998). He chairs the Health Insurance Study Group of Institut Montaigne, a think-tank on public policy sponsored by a group of major companies in France. After studying medicine and biology in Paris, Matthieu Lévi-Strauss obtained a PhD degree at the Pasteur Institute on genetics of the major histocompatibility complex. Nicolas Lévy is Professor of Human and Molecular Genetics. He is the head of the Medical Genetics department and of the Inserm research center “Medical Genetics and Functional Genomics”. In 2009, he was appointed as the director of the French national institute for rare diseases. He has been nominated as the head of the National Foundation for Rare Diseases, which will be created in 2011. Specialist in genetics of neuromuscular disorders and laminopathies, his team has provided the community with important achievements in these fields. After having identified the LMNA mutation causing Progeria, Nicolas Lévy’s efforts have been dedicated to the identification of genes and pathophysiological mechanisms involved in Progeria and related progeroid. Taking advantage of European collaborations, his laboratory has developed and pre-clinically tested a combination of global prenylation inhibitors in human cells and living mouse model. Based on the demonstrated efficacy of this combination, Nicolas Lévy is the PI of of the first phase II trial using amino-bisphosphonates and Statins for Progeria, in progress at La Timone Hospital in Marseille, France. Recently, his work dedicated to the understanding of mechanisms, and to the development of therapeutic strategies in neuromusccular disorders, led his team to demonstrate the proofs of principle for exon skipping and minigene transfer as potential therapies in dysferlinopathies. Jean-Louis Mandel, MD, PhD, born in 1946, is known for his work in human genetics, from the discovery of triplet repeat expansion as a mutation mechanism to the identification of genes implicated in about 12 other inherited neurological diseases. He has also worked on pathomecanisms of some of these diseases. He is team leader at IGBMC, a large biomedical research institute (CNRS, INSERM and Strasbourg University), of which he was director, then adjunct director from 2002 to 2009. Previously Professor of Genetics at the Faculty of Medicine of Strasbourg, he is since 2003 Professor of Human Genetics at the College de France. He is also head of the DNA diagnostic laboratory at the University Hospital in Strasbourg, which carries tests for about 30 monogenic diseases, and is one of the major diagnostic labs in France for neurological diseases. He is now particularly interested in developing applications of high throughput sequencing to diagnosis of genetically very heterogeneous diseases such as Bardet Biedl syndrome and related ciliopathies, ataxias and mental retardation. François Meyer est actuellement directeur de l’évaluation médicale, économique et de santé publique dans cette institution. Dr. François Meyer received his MD from the University of Montpellier Medical School in France in 1984. He is a qualified specialist in Endocrinology and Metabolic Disorders. From 1984 to 1992, he worked in Montpellier hospitals in the Endocrinology and Metabolic Disorders Unit, then in the Internal Medicine Unit. In 1992, he joined the R&D department of a swissbased international pharmaceutical company where he was employed as a medical sponsor for the first two years, and then as a Group Leader for oncology projects. In 1997, he became Deputy Director of the Medicines Evaluation Department at the French Medicines Agency (Agence Française de Sécurité Sanitaire des Produits de Santé - AFSSAPS). From 2000 to 2002, he was member, as the French representative, of the EMA’s Committee for Orphan Medicinal Products (COMP). In 2005 he joined a new public independent body, the French National Authority for Health (Haute Autorité de Santé, HAS). HAS was set up by the French government in order to bring together under a single roof a number of activities designed to improve the quality of patient care and to guarantee equity within the healthcare system. Depuis plus de 7 ans, elle s’occupe plus spécifiquement d’essais cliniques dans les maladies rares. Elle a été notamment l’investigateur principal du premier essai clinique réalisé chez les patients atteints de CMT1A, essai clinique mené sur 180 patients (Micallef et al, Lancet Neurology 2009) et est actuellement impliquée dans le premier essai thérapeutique industriel dans la CMT1A. L’expérience acquise au travers de cet essai et des autres actuellement en cours (Syndrome de Rett, Progéria,…) l’ont conduit à mettre en place une plate-forme dédiée aux spécificités des essais cliniques dans les maladies rares, OrphanDev. Orphandev a pour objectif de faciliter et d’accélerer la réalisation d’essais cliniques dans le domaine des maladies rares en proposant un support méthodologique et logistique aux chercheurs, cliniciens et industriels désireux d’évaluer un candidat médicament dans une maladie rare. D’abord soutenu en 2009 par le Centre National de Gestion des Essais de Produits de Santé (CeNGEPS) pour développer cette action au niveau régional, Orphandev étend son action au niveau national en 2011 grâce au renouvellement du soutien du CeNGEPS. Orphandev est appelé à rejoindre la Fondation Nationale Recherche Maladies Rares. Joëlle Micallef was born in 1969. After her training in the Marseille Medical School she obtained MD in Public Health in 1999. She qualified the Ph.D. in Neurosciences in 2002 and received the Post Doctoral Degree “Habilitation à Diriger des Recherches” in 2004. Since her nomination in 2002 as Associate Professor in Clinical Pharmacology she is performing numerous academic or industry promoted studies at the Clinical Pharmacology Unit of the Public Hospital in Marseille (AP-HM). For more than 7 years she has been specialized in clinical trials for Orphan therapeutics. She was prior investigator of the first clinical trials performed on CMT1A adults patients which recruited 180 volunteers (Micallef et al, Lancet Neurology 2009) and is associated investigator in the first industrial promoted study for CMT1A. Lessons learned from clinical trials in rare diseases (Neuromuscular diseases, Rett Syndrom, Progeria…) which specificities require to gather the strengths of each actor involved in research and patients’ care have led her to develop Orphandev. Orphandev is a national platform which aims at facilitating and accelerating the development of Orphan therapeutics. Orphandev brings methodological and logistical support to researchers, clinicians and industries willing to evaluate therapeutics in a rare disease indication. Orphandev has been supported since 2009 by the National Center for Health Products’ Evaluation (CeNGEPS, a public- private partnership between LEEM, Inserm and French Hospitals Federation to improve quality of French clinical research ) which entrusted it first at a regional level and then give Orphandev in 2011 the opportunity to help further at a national level. Christel is the mother of a 33 years old young woman born with a rare disease, at last diagnosed when she was 16 years old. Since then, she has been relentlessly fighting for diagnosis and access to care for people living with rare diseases across Europe. She was one of the founders of Prader-Willi France and of the French National Alliance for Rare Diseases. She coordinated two European conferences to raise awareness about rare diseases, in 2003 and 2005. She worked for several European projects and research networks. On behalf of EURORDIS, she participated in the preparation, the implementation and the evaluation of the first National Plan for Rare Diseases in France (2005-2008), then in the second Plan (2011-2014), also in the Communication of the European Commission “Rare Diseases, Europe’s challenge”(2008) and in the Recommendations of the Council for Rare Diseases (2009). Today she is a member of the European Committee of Experts on Rare Diseases and an advisor in the EUROPLAN project. She is also member of the Committee for Rights and Autonomy of people with disabilities in the region of Versailles, France, where she lives. My methodological research topics concern the design and analysis of adaptive and flexible clinical trials, the analysis of failure time data for competing risks and multivariate failure times, continuous monitoring of quality and performance and the analysis of observational studies using propensity score related methodologies. I was diagnosed in August 2004 after 4 years of repeated hospitalizations and physicians’ visits. My treatment was initiated in September 2004. My quality of life was improved after 6 months of treatment. An association of french patients was created in September 2005. At that time, there was no specific Fabry disease patient’s association in France. I’m the president of this association since its creation. I was reelected this year. There are 6 board’s members and a scientific comity of 6 physicians of different specialities, a psychologist and a assistant social. There are 220 members, 60 families all around France. We’ve developed an Internet Website: www.apmf-fabry.org, a facebook page and different documents (for adults, young people, pregnancy and Fabry disease, socials helps …) to inform patients. A patients’ testimonials video with information on the disease will also be achieved in the next months. He has been involved in the diagnostics of inherited diseases since 1994. The Center for Human Genetics is the largest genetic department in Belgium. The laboratory is offering molecular tests for the more common genetic diseases like cystic fibrosis, fragile X syndrome, Huntington’s disease, myotonic dystrophy, spinal muscular atrophy, hemophilia and Duchenne/Becker Muscular Dystrophy, and for a series of less frequent disorders. The laboratory also offers testing for familial breast- and ovarium cancer (BRCA1 and BRCA2) and familial colon cancer and other familial cancer syndromes. His major research interest is in Congenital Disorders of Glycosylation (CDG), a group of rare inborn errors of metabolism. Hughes-Wilson is one of the 4 industry members of the European Commission’s newly established EU Committee of Experts on Rare Diseases (EUCERD) and is Chair of the joint EBE and EuropaBio Industry Task Force on Orphan Drugs & Rare Diseases. Prior to joining Genzyme, Hughes-Wilson led the Emerging Biopharmaceutical Enterprises (EBE), a specialised group of the European Federation of Pharmaceutical Industries & Associations (EFPIA). EBE represents the interests of biotechnology companies in Europe. She has also worked in the animal health / veterinary medicines industry and for several years at Ernst & Young Consulting. Hughes-Wilson is a graduate in Law. Herwig Jansen graduated in 1984 as a medical doctor in the medical school of the University of Ghent (RUG) Soon afterwards he started as a general practitioner in a rural village in the east of Belgium. In 2000 he graduated in Public Health, specialized in children’s public health and epidemiology. He combined his private practice as a general doctor with teaching epidemiology and and social medicines at the Vrije Universiteit Brussel. During this period he became involved with the scientific project creating the National cystic fibrosis registry (BMRRBM) in Belgium.
