Formulaire de demande de participation
Après avoir complété le formulaire de demande de participation, les candidats doivent soumettre leur dossier complet au responsable de leur établissement d’attache selon la date fixée par l’établissement (31 janvier 2013) Envoyer a: Oficina de Relaciones Internacionales, Biblioteca 5ª Planta, 48940 Leioa, Bizkaia. Att. programa CREPUQ Le responsable de l’établissement d’attache assurera la transmission des dossiers aux établissements d’accueil québécois. Une demande de participation transmise directement par un candidat ne sera pas considérée. Le dossier de candidature doit comprendre les pièces suivantes: 1. une copie imprimée du formulaire électronique de demande de participation dûment signée par le candidat ; 2. une copie de la fiche individuelle d’état civil ou de tout 4

3. une copie du relevé de notes attestant les cours universitaires complétés et la liste des cours auxquels le candidat est présentement inscrit à l’établissement d’attache ; 4. une lettre, rédigée dans la langue d’enseignement de l’établissement d’accueil, présentant les objectifs de formation poursuivis en participant au programme d’échanges ; 5. la liste des cours (sigle et titre) que le candidat projette de suivre ; cette liste doit être dressée pour chacun des établissements d’accueil retenus ; 6. une approbation de la liste des cours projetés émise par le doyen, le directeur des études ou l’instance appropriée de l’établissement d’attache. Cette liste doit être approuvée pour chacun des choix d’établissement ; 7. une lettre personnalisée de recommandation émise par le doyen, le directeur des études ou l’instance appropriée de l’établissement d’attache ; 8. une lettre émise par le doyen, le directeur des études ou l’instance appropriée attestant la maîtrise de la langue d’enseignement de l’établissement d’accueil, sauf si le programme d’études projeté porte sur l’étude de cette 5

3. a copy of the marks received for university courses completed and a list of the courses in which the candidate is currently registered at the home institution 4. a letter, written in the language used at the host institution, stating the educational objectives of participation in the exchange program 5. the list of courses (with numbers and titles) that the candidate plans to take; this list should be compiled for each host institution selected 6. approval by the dean, program director or appropriate official at the home institution of the list of planned courses for each of the universities selected 7. a personal letter of recommendation from the dean, program director or appropriate official of the home institution 8. a letter from the dean, program director or appropriate official certifying the candidate’s proficiency in the language used at the host institution, unless the planned study program involves studying that language. UNIVERSITÉS DU QUEBEC / QUEBEC UNIVERSITIES: INSTRUCTIONS/INSTRUCTIONS 2013/14 NOTES: Les participants aux programmes d’échanges de la CREPUQ devront s’inscrire à un régime d’études à plein temps. Le régime des études varie selon les établissements universitaires, mais habituellement, les participants s’inscrivent à 12 crédits (quatre cours) ou 15 crédits (cinq cours) par session. Généralement, un cours exige trois heures de présence en classe par semaine, durant 15 semaines ; cela ne comprend pas la charge de travail personnel (lectures, travaux, étude, etc.) qui équivaut à environ six heures de travail par semaine par cours. Les participants aux programmes d’échanges de la CREPUQ s’inscrivent à des cours offerts par l’établissement d’accueil. L’encadrement d’activités de recherche ou de stages ne sera pas assuré par l’établissement d’accueil, mais par l’établissement d’attache. NOTES: CREPUQ student exchange participants must register for a full‐time course of study. Study programs vary with the university, but usually participants register for 12 credits (four courses) or 15 credits (five courses) per session. Generally, a course requires three hours of class attendance per week for 15 weeks; this does not include the personal work load (reading, assignments, review, etc.) which is about six hours of work per week for each course. CREPUQ student exchange program participants register for courses offered by the host institution. Supervision of research or internships will not be done by the host institution, but by the home institution. 7
