PROBLÉMATIQUE

Schizophrénie : 1% population Québec (MSSS,2013).

Maladie mentale la plus invalidante (Townsend, 2010) 
Prise d’antipsychotiques (1e et 2e générations) (Townsend, 2010)
qui améliorent la qualité de vie, mais entrainent plusieurs effets secondaires (Ascher‐Svanum et al., 2006).
Non observance au Québec : entre 23 % (Ward, Ishak, Proskorovsky & Caro, 2006) et 50% ( Lee, Kane, Sereika, Cho & Jolley, 2011) des schizophrènes. 
FACTEURS DE RISQUE DE NON-OBSERVANCE

Présence de symptômes positifs (Stuart et Laraia, 2005).

Effets secondaires (anticholinergiques, métabolique…) (Gray, White, Schulz & Abderhalden, 2010; Stuart et Laraia, 2005).

Absence d’alliance thérapeutique (Stuart et Laraia, 2005).

Éducation inadéquate du patient et de la famille (Stuart et Laraia, 2005).

Polymédication, antécédents de non adhérence (Stuart et Laraia, 2005).

Alcoolisme et toxicomanie (Stuart et Laraia, 2005).: environ 80% (ASPC, 2002).

CONSÉQUENCES DE NON-OBSERVANCE

Persistance des symptômes positifs et négatifs (Davis, Chen & Glick, 2003). 
Jusqu’à 40% de rechute (Passerieuxa Caroli & Giraud‐Baroc, 2009)

Plus de 50% des réhospitalisations

Rechutes successives peuvent causer une résistance aux Rx (Olivares, (Scholer, 2003; Pitschel‐Walz et 2006).
Alptekin, Azorin, & al., 2013).

Non rétablissement du patient
( Blondeau, Nicole, & Lalonde, 2006; Davis, Chen & Glick, 2003). INTERVENTIONS EFFICACES

Télénursing : (Beebe, Smith, Crye, Addonizio, Strunk, Martin, & Poche ,2008).



Appels téléphoniques 1fois/semaine
Augmente adhérence de 20%
Dépister tôt et traiter les effets secondaires des Rx (Puschner et al., 2009; Varcarolis et Halter, 2010).

Impliquer la famille dans toute la prise en charge (Varcarolis et Halter, 2010).

La psychoéducation faite auprès du patient et de sa famille ( Hervieux et al.,2007).

Adapter l’information selon la compréhension du patient (Beauchamp, Lévy‐Chavagnat, & Chavagnat, 2013).


Renforcement positif et soutien par les pairs (Varcarolis et Halter, 2010).

Entrainement cognitif adapté (Velligan et al., 2000; Velligan et al., 2008).
Advocacy de l’infirmière pour suggérer au MD de: simplifier les prises, recourir aux Rx de longue action (Gourevitch, 2006; Puschner et al., 2009) .


Entretien motivationnel ( Beebe et al.2008; Phaneuf, 2006 ).
INTERVENTIONS NON EFFICACES
‐ Blâmer le patient sans investiguer les raisons (Puschner et al.. 2009; Stuart et Laraia, 2005).
‐ Paternalisme et l’empathie excessive (Charpentier, Goudeman & Thomas, 2009).
‐ Enseignement basé sur la peur et les menaces (Phaneuf, 2006).
