TREATMENT OF ENDOMETRIOSIS RECURRENCE. CLINICAL CASE. Fifty-five percent of women with ovarian endometriosis concurrently have endometrioma. Surgery does not always lead to recovery, thus further rehabilitation therapy is required. Objective: To evaluate the effectiveness of endometriosis treatment with dienogest. Materials and Methods: Patient K., 36 years, suffers from endometriosis within last 5 years. Surgical treatment was performed twice: 1) in 2009, laparoscopy, left adnexectomy, excision of endometriotic lesions. Rehabilitation therapy after surgery was not performed. 2) in 2010, laparotomy, hysterectomy with right ovariectomy because of adenomyosis, uterine fibroids, right ovarian mass. Histological findings: 1) endometrioid ovarian cyst (2009); 2) adenomyosis, nodular form, uterine myoma with cystic degeneration, mixed hyperplastic endometrium, corpus luteum cyst. After 1 year, the patient complaints on frequent urge to urinate. Pelvic ultrasound (31.10.11): condition after hysterectomy, in the projection of the lower third of the right ureter liquidfilled structure 26,5 x 21,5 x 19,2 mm, volume - 5.7 cm3. Renal ultrasound (31.10.11): right pyelectasis, microlithiasis of both kidneys, chronic cystitis, urethritis, hypotonic bladder. Excretory urography (31.10.11): no pathology of the urinary system. Pelvic MRI (4.11.11) revealed liquid-filled structure behind the bladder with hyperintense signal and size of 1,8 x 2,0 mm. Pelvic sonography (06.01.12) showed increased size of the pelvic mass up to 32x34x28,6 mm; volume 16.8 cm3. Considering growth of the mass surgical treatment was recommended, but the patient refused. Dienogest was prescribed in dose of 2 mg per day for 6 months. Results: after 12 and 24 weeks of therapy no pelvic mass is visualized at sonography. Conclusion: This case demonstrates high therapeutic efficacy of dienogest, its use prevented repeated surgery.
