The types of forms used are hereafter identified in short by the letters:

STATEMENT OF INCOME AND EXPENSES - 6 to 11 dwellings
1 - OWNER OCCUPIES ENTIRE PROPERTY OR PART OF IT
a)
If the property is ENTIRELY or PARTIALLY being used by the owner or/and a legal person under his control ,
use the box OWNER OCCUPANT on the top of the RD form to enter the percentage of total property occupied
by the owner or the parent entity.
* If this percentage is 100 % , just skip the INCOME section.
* If percentage of occupation is less than 100 % , the INCOME section should be used to show the rents and
other income collected from any tenant OR write the mention "NO INCOME COLLECTED" .
* In all cases , however, the OPERATING EXPENSES section must be completed.
b)
Correct or complete PRE-PRINTED INFORMATION concerning the property found on the forms, by writing
directly on the forms.
2 - PROPERTY IS NOW 100% RESIDENTIAL
a)
5 HOUSING UNITS OR LESS: PLEASE RETURN all the documents with a note mentioning on which date the
property became 100 % residential.
b)
6 HOUSING UNITS to 11: you must complete the form LD2D.
12 HOUSING UNITS OR MORE: you must complete the forms RD02 and LL02 .
If you did not receive those forms in our mailing, please call 280-8964 (voice mail) to ask for them as soon as possible.
Those forms are also available on our website at www.ville.montreal.qc.ca/evalweb under the section
"Our documents and forms - Collection of economic data"
3 - THE PROPERTY IS FOR SALE
Please complete the enclosed forms and send us the broker's inscription, copy of the MLS or copy of the complete
" for sale" prospectus.
4 - YOU HAVE SOLD THE PROPERTY
If you no longer own the property, please use the return envelope to mail back
1 - All the enclosed forms
2 - A list of rents and tenants or occupants and financial statements used to put the property for sale
3 - A copy of the M.L.S. listing, if any, or a note giving the name of the broker who was instrumental in selling your
property on an exclusive mandate
4 - Also mention the registry number and date of the deed of sale.
N.B. Data on recently sold property are of particular interest to the assessor since they allow him to relate property operating
data to sale prices, and thus better understand the workings of the market.

5 - YOU JUST BOUGHT THE PROPERTY
If you just acquired the property, please complete the forms on the basis of the operating data provided by the vendor at
the time of sale, with a note stating clearly the source of your informations. If it is impossible for you to provide the
past operating data, please enter on the forms the forecasted income and expenses you had in mind at the time you
acquired the property. On the forms, mention clearly that such data are "ESTIMATED".
6 - THE PROPERTY WAS REPOSSESSED
If the property was repossessed, please return all the forms in the return envelope with a note stating the registry number
and date of the deed of sale or assignment.
7 - FINANCIAL STATEMENTS IN LIEU OF COMPLETED FORMS
Financial statements, preferably audited, or other prepared documents could be returned to us in lieu of the completed
forms. If these documents contain all the informations required on the forms, you do not have to complete the forms. If they
do not contain all the informations required, send them anyway and use the forms to supplement missing data.
In any case, you MUST return all the forms received. Please sign and complete your contact information on the first page of
the RD form (INCOME & EXPENSES form) and make a clear reference to the documents forwarded.
8 - "CONSOLIDATED" FINANCIAL STATEMENTS OR DATA
If your audited financial statements are "consolidated", meaning that data on more than one property are grouped together
(often for fiscal purposes), then please use the forms or send any other document to present the individual performance and
data of EACH property. Preliminary worksheets, annexes, computer printouts, etc, even unaudited , will do.
9 - LIST OF RENTS AND TENANTS OR OCCUPANTS - (LL)
You can make corrections directly on the forms and/or add information if needed. If you already have a list of tenants or
other documents containing the information asked on the forms, send them to us. In that case, you need not fill out the
forms. To obtain additional forms, follow the procedure in point 10.
* In any case, you MUST return all the forms received. Please sign and complete your contact information on the first page
(LIST OF RENTS AND TENANTS OR OCCUPANTS) and make a clear reference to the documents forwarded.
10 - REQUEST FOR ENGLISH FORMS
To obtain english forms, please call 514 280-8964 (voice mail) and follow instructions. LEAVE YOUR NAME AND
COMPLETE MAILING ADDRESS AND MENTION THE ACCOUNT NUMBER , THE CATEGORY OF PROPERTY AND
THE DIVISION. We will forward you the english forms as soon as possible.Those forms are also available
on our website at www.ville.montreal.qc.ca/evalweb under the section " Our documents and forms - Economic data"
Whether you complete the french (if owner is a company or if you decide to do so) or the english version of the forms, do not
forget to ENCLOSE BOTH SETS of forms in the return envelope. At the time of the next collection of economic data, IF THE
OWNER OF THE PROPERTY, ACCORDING TO THE ASSESSMENT ROLL, IS A PERSON OTHER THAN A COMPANY,
english forms will be sent right away.
11 - DEMANDE DE FORMULAIRES EN FRANÇAIS
Pour obtenir des formulaires en français, composez le 514 280-8964 (boîte vocale) et suivez les instructions. LAISSEZ
VOTRE NOM ET VOTRE ADRESSE POSTALE COMPLÈTE ET MENTIONNEZ LE NUMÉRO DE COMPTE ET LA
CATÉGORIE DE LA PROPRIÉTÉ AINSI QUE LA DIVISION CONCERNÉE. Les formulaires en français vous seront postés
dans les plus brefs délais. Vous pouvez également imprimer ces formulaires en accédant à notre site Internet
(www.ville.montreal.qc.ca/evalweb) sous la rubrique : «Nos documents & formulaires - Collecte de données économiques»
Complétez les formulaires en français et identifiez la propriété visée (numéro de compte et adresse de la propriété).
Retournez le tout dans l'enveloppe de retour, en incluant les formulaires en anglais reçus en premier lieu. Lors de la
prochaine collecte de données économiques, vous recevrez automatiquement des formulaires en français.
