Complete the agreement with one of Summit’s staff. 2. You may pick up your new card at Summit’s office or it can be mailed to you. Please review the card’s fee schedule before signing the agreement.
The card DOES NOT have a Visa or MasterCard logo. It may not be accepted at some locations such as
restaurants, hotels, and other places where PIN based transactions are not accepted. Many cell phone
carriers ACCEPT over the phone payments using the Life Freedom Card.
A note will be added to your check stub as soon as the program becomes
available. At that time, you can check back here for instructions detailing how
to apply for a card or you can call Summit’s office for information. The exact
date is not known at this time; so, please do not call until you see the note.
