Laporte, Institut de Veille Sanitaire, Saint-Maurice, France es articles présentés dans ce numéro d’Eurosurveillance sur l’évolution récente des comportements de prévention chez les homosexuels masculins en Espagne, en Suisse, et aux Pays-Bas font le même constat d’un recul de la pratique du safer sex. Ils mettent en évidence une augmentation de la pratique de la pénétration anale quel que soit le type de partenaire, une augmentation des rapports anaux non protégés et de l’éjaculation dans la bouche, en particulier avec des partenaires occasionnels. L’augmentation des diagnostics de maladies sexuellement transmissibles aiguës (gonococcie, syphilis) dans différents pays d’Europe, illustrée dans ce numéro par les données des Pays-Bas, est la première conséquence objectivable de cette évolution. he articles published in this issue of Eurosurveillance on recent trends of preventive behaviours in homosexual men in Spain, Switzerland, and the Netherlands, all conclude that the practice of safer sex is declining. They emphasise increases in anal penetration (regardless of the nature of the relationship), nonprotected anal intercourse, and ejaculation in the mouth, in particular among casual partners. The increase of diagnoses of acute sexually transmitted diseases (such as gonorrhoea and syphilis) in different European countries – illustrated in this issue by the data from the Netherlands – is the first tangible consequence of this decline. L’impact négatif possible des nouvelles associations thérapeutiques antirétrovirales sur la prévention avait été évoqué dès 1996, immédiatement après l’annonce de leur efficacité. De nombreuses études ont depuis été menées sur ce sujet. Les études sur les connaissances, attitudes et comportements sexuels dans le nouveau contexte thérapeutique restent peu convaincantes. Celles publiées depuis 1997 (1,2) trouvent des résultats très similaires, c’est à dire une excellente connaissance des intérêts et limites des nouveaux traitements et une minorité d’individus (moins de 10 % des répondants) qui déclarent moins pratiquer le safer sex du fait des possibilités thérapeutiques. Les individus constituant cette minorité, ne sont-ils pas ceux qui prenaient régulièrement des risques avant l’arrivée des nouveaux traitements, justifiant ainsi à posteriori leurs comportements ? Les études réalisées sur des cohortes de patients dont certains sont traités sont plus démonstratives d’un ➤ Concerns about a possible negative impact of the new combined antiretroviral drugs (HAART) on prevention were raised in 1996 immediately after their efficacy was proved and numerous surveys on this subject have been carried out since then. Surveys on knowledge, attitudes and sexual behaviour in the new therapeutic context remain unconvincing. The studies published since 1997 (1,2) have similar results, namely an excellent understanding of the benefits and limits of new treatments and a minority of people (less than 10% of all respondents) reporting a decreasing practice of safer sex because of treatment possibilities. Perhaps the people in this minority were those who regularly took risks before the introduction of new treatments, thus justifying their behaviours with hindsight? Il ressort que le passage d’une charge virale détectable à une charge virale indétectable est un facteur d’augmentation des prises de risque (Stolte) quoique limité dans le temps. Par ailleurs, si l’on compare des homosexuels et des hétérosexuels recevant un traitement puissant, le risque de rapports non protégés est trois fois supérieur chez les homosexuels masculins par rapport aux hétérosexuels (3). Si l’on retourne 10 ans en arrière, on retrouve une augmentation des gonococcies chez les homosexuels aux Etat-Unis en 1989 (4), dans différents pays d’Europe entre 1989 et 1990 (Royaume-Uni (5), Pays-Bas (6)), une augmentation de l’incidence de l’infection VIH dans une cohorte d’homosexuels aux Pays-Bas en 1990 (6). Les nombreux articles sur une reprise inquiétante des comportements à risque et la crainte d’une augmentation de la transmission du VIH faisaient alors référence à l’arrivée de nouvelles générations n’ayant pas connu les effets dévastateurs de l’épidémie de SIDA et à la difficulté de maintenir au fil des années des comportements de protection. L’histoire se répète, les interprétations changent. Si certaines des études publiées dans ce numéro indiquent que les changements de comportements liés à l’apparition des nouveaux traitements sont particulièrement nets chez les séropositifs, on ne peut toutefois pas faire porter aux nouveaux traitements tout le poids de ce relâchement de la prévention et passer sous silence ces autres interprétations. Il s’agit à présent de mettre en œuvre des études plus qualitatives sur les motivations du maintien ou de l’abandon des comportements de protection pour tenter de donner à la prévention un contenu adapté au nouveau contexte et repousser la possibilité d’une reprise de l’épidémie. On remarquera enfin que les quelques études disponibles portent essentiellement sur les homosexuels masculins, et que les reculs de la prévention susceptibles de se produire dans d’autres groupes, chez les usagers de drogue en particulier, restent pour l’instant très mal étudiés. ■ References ➤ among HIV positive men (3, Stolte presented here). It seems that the change from a detectable viral load to an unde-tectable viral load is a factor of increased risk taking (Stolte), albeit temporary. Moreover, if we compare homosexuals and heterosexuals receiving HAART, the risk of unprotected intercourse is three times higher in homosexual men than in heterosexual men (3). Ten years ago, there was an increase of gonococcal infections in homosexual men in the United States in 1989 (4) and in various European countries between 1989 and 1990 (United Kingdom (5), Netherlands (6)), and an increase of the incidence of HIV infection in a cohort of homosexual men in the Netherlands in 1990 (6). The numerous articles on the worrying emergence of high risk behaviour and the fear of increased HIV transmission were interpreted as younger generations ignorant of the devastating effects of the AIDS epidemic and the difficulty in maintaining protective behaviours over the years. History repeats itself, but interpretations change. Although some of the studies in this issue indicate that behavioural changes linked to the introduction of new treatments are particularly clear in seropositive homosexual men, we cannot attribute the drop in prevention to these new treatments and ignore possible other interpretations. It is necessary to set up more qualitative studies on the motivating factors for maintaining or abandoning preventive behaviours. This will allow prevention strategies to adapt to a new context, and delay the possible emergence of epidemics. It is worth mentioning that the few available studies deal essentially with homosexual men and that the decline of prevention that is likely to occur in other groups, mainly drug users, is currently not being studied to any extent. ■ 1. Although the latest survey on prevention behaviours of men having sex with men (MSMs) in Switzerland shows a decreasing number of HIV cases since 1992, a decrease in the protection level has also been observed. This could lead to a higher rate of HIV notifications in 2001 compared to 2000. ix enquêtes (1987, 1990, 1992, 1994, 1997, 2000) (1-3) ont permis à une équipe de chercheurs suisses de faire un bilan sur 13 ans des comportements de prévention des HSH en Suisse. La population homosexuelle a été, dès le début de l’épidémie, particulièrement touchée dans ce pays, qui affichait, à la fin des années 80, les taux de prévalence de cas de Sida les plus élevés en Europe (5). ix surveys (conducted in 1987, 1990, 1992, 1994, 1997, and 2000) (1-3) have enabled a team of researchers in Switzerland to assess preventive behaviour among men who have sex with men (MSMs) over the past 13 years. The surveys were carried out as part of the evaluation of the AIDS prevention strategy in Switzerland (4). Since the beginning of the epidemic, the homosexual population has been particularly affected in this country, which in the late 1980s had the highest prevalence of AIDS in Europe (5). Although the response rate cannot be calculated because of the wide distribution of the questionnaire, it is probably under 10%. On note toutefois une évolution dans la distribution des classes d’âge, avec un léger vieillissement de l’échantillon dans les dernières enquêtes. Pour simplifier la présentation des résultats nous avons choisi de montrer les données agrégées en signalant d’éventuelles différences selon l’âge. The main sociodemographic trends of the samples changed little between 1987 and 2000. We have, however, observed developments in age group distribution, with a slightly older sample in the most recent surveys. We performed analyses by stratifying data by age. To simplify the presentation of the results, we are showing aggregate data and point out potential differences by age. Activité sexuelle Certaines caractéristiques de la vie sexuelle des HSH n’ont pas changé depuis 1987. Le nombre de partenaires est élevé : la médiane du nombre de partenaires dans les douze derniers mois se situait dans la catégorie 2-5 jusqu’en 1990, elle est entre 6 et 10 et reste stable dès 1992. Les trois quarts des répondants ont eu plus d’un partenaire au cours des 12 derniers mois et plus d’un tiers ont eu plus de 10 partenaires. La fréquence des relations sexuelles est élevée : plusieurs fois par semaine pour un tiers des répondants. D’autres caractéristiques montrent des changements avec une évolution ascendante culminant en 1997. La proportion de répondants ayant un partenaire stable (que cette relation soit exclusive ou non) a augmenté : 46% en 1987, 54% en 1992, 74% en 1997, 72% en 2000. La proportion de répondants qui pratiquent la pénétration anale avec leurs partenaires occasionnels au cours des douze derniers mois a augmenté : 60% en 1992 et en 1994, 69% en 1997 et en 2000 mais elle n’a pas changé avec le partenaire stable (environ 75%). La fréquentation des ➤ Sexual activity Some trends in sexual activity among MSMs have not changed since 1987. The number of partners is high: the median number of partners in the latest 12 months was two to five until 1990, six to ten in 1992 and remained the same since then. Three quarters of respondents had more than one partner in the last 12 months, and over one third had more than 10 partners. Sexual intercourse is frequent: one third of respondents had sex several times a week. Other trends show further changes, with significant increases that peaked in 1997. The proportion of respondents with a longterm partner (whether the relationship is exclusive or not) has increased from 46% in 1987, 54% in 1992, 74% in 1997, to 72% in 2000. The proportion of respondents who practice anal penetration with casual partners in the past 12 months has increased: from 60% in 1992 and in 1994, 69% in 1997 and in 2000, but it has not changed between long term partners (around 75%). The proportion of men reporting visits to pickup places has increased: ➤ EUROSURVEILLANCE VOL. 7 - N° 2 FÉVRIER - FEBRUARY 2002 17 ➤ lieux de drague a augmenté : en 2000, 53% des HSH fréquentent les saunas (30% en 1987, 53% en 1997) ; 37% les parcs (respectivement 24% et 41%) ; 26% les toilettes publiques (respectivement 18% et 27%). Comportements préventifs Les comportements préventifs ont été mesurés à l’aide de deux indicateurs : l’usage du préservatif lors de la pénétration anale et le fait de recevoir du sperme dans la bouche lors de la fellation, durant les douze derniers mois (tableau). Dès 1987, on constate des niveaux élevés de comportements préventifs soit par abstention de rapports sexuels anaux ou oraux, soit par utilisation de préservatifs, tous partenaires confondus. Par la suite, le comportement préventif a été recensé selon la nature de la relation. Il en résulte que plus de 80% des HSH se protègent systématiquement lors de rapports sexuels avec un partenaire occasionnel, bien que le niveau de protection ait diminué ces dernières années, quelle que soit l’option de prévention adoptée. Avec le partenaire stable, tant l’abstention de pénétration que l’usage de préservatifs sont inférieurs et la situation a peu changé depuis le début des années 90. A noter que la plupart des couples qui n’utilisent pas de préservatifs sont tous deux séronégatifs (6). L’évolution est semblable chez les moins de 30 ans et les 30 ans et plus. Test VIH et séropositivité rapportés En 1987, 57% des HSH avaient été dépistés au moins une fois pour le VIH. A chaque étude, la proportion d’HSH testés s’est élevée et elle a atteint 80% en 2000. La proportion d’HSH qui se disent séropositifs a légèrement diminué, quoique de façon non significative, passant de 14% en 1987 à 11% en 2000. En 2000, la prévalence rapportée est de 2% chez les moins de 30 ans et de 12% chez les 30 ans et plus. Discussion Il est impossible d’apprécier la représentativité de ce type d’échantillon auto-sélectionné. On peut faire l’hypothèse que les répondants, en tant que lecteurs de journaux gays ou membres d’associations, appartiennent à la couche la plus affirmée du point de vue de son identité, la plus informée et la plus motivée de la population, afin d’éviter l’infection dans la société. Les jeunes sont également peu représentés dans ce type d’enquête. Il a aussi été démontré que les études faisant appel à ce type de recrutement sur-représentent les HSH avec un niveau d’éducation plus élevé (7,8). Les niveaux de protection sont probablement biaisés vers le haut - bien qu’une stratification des données selon le niveau d’éducation n’ait pas montré de différence significative dans notre échantillon (9). Néanmoins, la bonne stabilité des caractéristiques sociodémographiques des échantillons autorise l’analyse des grandes tendances. On constate donc que les comportements préventifs restent largement majoritaires dans cette population. La légère tendance à la baisse observée ces dernières années doit cependant appeler à renforcer la vigilance. Les données du système de surveillance du VIH ont montré que les nouveaux cas de VIH déclarés ont diminué régulièrement depuis 1992 dans tous les groupes de population. Toutefois, en 2000 on observait un tassement de cette tendance et les nouvelles déclarations de VIH chez les hommes en 2001 pourraient être plus nombreuses qu’en 2000 au vu de l’évolution au premier semestre (10). Ceci justifie pleinement le maintien de moyens importants destinés à la prévention du VIH/Sida chez les HSH. ■ References 18 ➤ in 2000, 53% of MSMs visited saunas (30% in 1987, 53% in 1997); 37% parks (24% and 41%, respectively), and 26% public toilets (18% and 27%, respectively). Preventive behaviour Preventive behaviour has been calculated by using two indicators: wearing a condom during anal penetration and sperm in the mouth during oral sex in the last 12 months (table). As early as 1987, high levels of preventive behaviour were observed – either through abstention of anal or oral sex or use of condoms –- all types of partners considered together. Subsequently preventive behaviour was recorded according to the nature of the relationship, and we found that over 80% of MSMs systematically protected themselves during sexual intercourse with a casual partner, although in recent years levels of protection have decreased, irrespective of the prevention option adopted. With a long term partner, both abstinence from penetration and the use of condoms are lower and the situation has changed little since the early 1990s. It is worth mentioning that most couples who do not use condoms are both seronegative (6). The trend is similar in those younger than 30 years and those aged 30 and over. Reported HIV test and seropositivity In 1987, 57% of MSMs were tested at least once for HIV. In each survey the proportion of tested MSMs increased, and reached 80% in 2000. The proportion of MSMs who report to be seropositive has slightly, but not significantly, decreased from 14% in 1987 to 11% in 2000. In 2000, the reported prevalence was 2% in those under 30 and 12% in those aged 30 and over. Discussion It is impossible to appreciate how representative this type of self selected sample is. We can assume that the respondents – readers of gay magazines or members of gay associations – belong to the most identity affirmed, the most informed and the most highly motivated to avoid infection strata of society. Young people are also underrepresented in this type of survey. It has been shown that surveys using this type of recruitment overrepresent MSMs with higher levels of education (7). Protection levels are probably biased upwards – despite the fact that data stratification (into categories – primary and higher levels) according to the education level has shown no significant difference in our sample (9). The good stability of the sociodemographic characteristics of the samples, however, allows main trends to be analysed. Preventive behaviours are widely practised in this population, according to the findings of the surveys. The slight downward trend, however, that has been observed over the past few years calls for reinforced vigilance. Data from the HIV surveillance system have shown that the number of new reported cases has continuously decreased since 1992 in all age groups. However, in 2000 a downturn was observed for this trend, and in 2001 the number of new HIV notifications for men could be higher than in 2000, considering the progression of new cases in the course of the first six months (10). Standardised data about sexual behaviour and the occurrence of sexually transmitted diseases (STDs) have been collected using self administered questionnaires, and blood was drawn for HIV testing. In addition we have collected data about STD diagnoses from the STD outpatient clinic of the municipal health service. These data were used for the two studies summarised in this article (1,2). Plusieurs études ont d’ailleurs établi l’existence d’un tel lien (7-9). Dans les services de consultation externes d’un centre de santé pour MST à Amsterdam, on a observé une très forte augmentation du nombre de cas de gonorrhée rectale et de syphilis précoce (10). Cette inquiétante tendance au développement des MST à Amsterdam et ses possibles implications en terme de développements de comportements sexuels à risque ont été analysées dans deux études portant sur des hommes homosexuels et bisexuels. Ces études se sont principalement intéressées aux liens pouvant exister entre ce phénomène et la mise sur le marché des HAART. In San Francisco, an increase in gonorrhoea in 1997 was followed by an increase in the number of HIV infections in 2001 (6). The increase in STDs is probably caused by an increase in risky sexual behaviour. The question is, why this increase has occurred in recent years. A possible explanation could be that people have no longer found it necessary to practise safe sex since Highly Active Antiretroviral Therapies (HAART) became available in 1996. Several studies have shown such a link (7-9). In the STD outpatient clinic in Amsterdam, the number of cases of rectal gonorrhoea and early syphilis has increased dramatically (10). This worrying trend in STDs in Amsterdam and its possible underlying change in risky sexual behaviour was investigated in two studies among homo- and bisexual men, which concentrate on a possible relation with the introduction of HAART . Sexually transmitted diseases and anti-HIV drugs To determine independent risk factors for STDs before and after the introduction of HAART, and to investigate a possible association between the increase in STDs and the introduction of these therapies, we reviewed data from a group of homo- and bisexual men attending the STD outpatient clinic of the municipal health service in Amsterdam between 1994 and 1999 (1). Parmi les hommes homosexuels VIHnégatifs qui déclaraient avoir eu des relations anales, la fréquence des rapports non protégés avec des partenaires réguliers était passée de 70% (en 1996) à 78% (en 1999). Cette même fréquence était passée, avec des partenaires occasionnels de 28% à 33%. Nous avons également observé, en s’appuyant sur des données indépendantes, une hausse des cas de gonorrhée chez les jeunes hommes homosexuels séropositifs VIH. Le nombre de nouveaux cas de gonorrhée après juillet 1996 avait presque atteint le nombre de cas observé au milieu des années 1980 (figure 4). Le nombre de nouveaux cas d’infections à VIH ne semble cependant pas avoir encore augmenté. Il convient de souligner que les changements observés dans les comportements sexuels et dans l’incidence des infections transmises par voie sexuelle coïncident bien avec l’introduction sur le marché des nouveaux traitements anti-VIH. Cela ne constitue cependant pas une preuve irréfutable que ces changements sont directement imputables à l’apparition de ces nouvelles thérapies. Les auteurs d’une autre étude (2), menée auprès d’hommes homosexuels séropositifs VIH de tous âges (moyenne d’âge de 35 ans), ont ainsi essayé de savoir dans quelle mesure le comportement sexuel à risque était influencé par la prise ➤ new cases of gonorrhoea after July 1996 almost reached the level of the mid 1980s (figure 4). The number of new cases of HIV infections does not seem to have increased yet. It is noteworthy that the changes in sexual behaviour and STDs coincide with the introduction of the new anti-HIV drugs. This, however, is no proof that the changes are attributable to these new drugs. A separate study (2) among HIV positive homosexual men of all ages (mean age 35 years) therefore examined the extent to which the sexual risk behaviour is influenced by the actual administration of the drugs. From this study it became apparent that it was not so much the administration of the drugs, but rather the virological and immunological improvements resulting from them, that influences the practising of unsafe sex. After the CD4 cell count numbers increased and the viral load dropped below the detection level, unsafe sex with casual partners was practised more often. It may be that this increase in risky sexual behaviour is of short duration because the frequency of unsafe behaviour was lower again with the sustained virological and immunological improvements seen with continued treatment. But follow up time since treatment was short. Conclusion The combined results of the two studies summarised in this article indicate that an increase in STDs and risky sexual behaviour is currently occurring among homosexual men in Amsterdam. The introduction of highly active antiretroviral therapies may have had an influence on these increases. Le nombre de nouveaux cas d’infections à VIH n’a pas encore augmenté, et il est difficile de déterminer comment et si cette tendance au développement de comportements sexuels à risque, va se traduire prochainement par une augmentation du nombre de cas d’infections à VIH. L’incidence du VIH dépend en effet de nombreux facteurs qui peuvent avoir plusieurs types d’influence différents (13). Les études présentées dans cet article montrent également qu’au moment où de nouvelles solutions thérapeutiques sont proposées, il est nécessaire de renforcer les programmes de sensibilisation à des pratiques sexuelles moins risquées, destinés aux hommes homosexuels. Jusqu’en décembre 2000, 13 275 cas de Sida ont été déclarés et le taux annuel d’incidence du Sida s’élevait à 81,8 par million d’habitants en 2000. Les hommes homosexuels représentaient, en tant que groupe de transmission, 20% de tous les cas de Sida pour l’année 2000, soit une légère augmentation par rapport à 1998 (17,6%) (1). La surveillance du VIH et des comportements à risque parmi les hommes ayant des relations sexuelles avec d’autres hommes (HSH) a été mise en place en 1993 (2), dans le cadre du système de surveillance intégré catalan pour le VIH/Sida. atalonia is an autonomous region in Spain with six million inhabitants. By December 2000, 13 275 cases of AIDS had been reported and the annual AIDS incidence rate was 81.8 per million in 2000. As a transmission group, homosexual men represented 20% of the total number of AIDS cases in 2000, showing a slight increase since 1998 (17.6%) (1). As part of the Sistema integrat de vigilància epidemiològica de l’HIV/sida a Catalunya (Catalan integrated surveillance system of HIV/AIDS), monitoring of HIV and sexual risk behaviour among men who have sex with men (MSMs) was introduced in 1993 (2). Différentes études chez les HSH ont récemment révélé une augmentation de l’incidence du VIH (3-5), et des infections sexuellement transmissibles (6-8), de même qu’une augmentation des comportements à risque (9,10). L’évolution des comportements sexuels à risque est en partie expliquée par l’optimisme concernant les traitements du VIH et l’absence de menace de mort telle qu’elle existait, il y a dix ans. This change to unsafe sexual behaviour has been explained in part by the optimism surrounding HIV treatment and the absence of the threat of death that existed a decade ago. Some authors found a significant relationship between unprotected anal intercourse and certain aspects of optimism in the context of new HIV treatments (11-12). L Les objectifs de cet article sont de décrire les tendances de la prévalence de l’infection à VIH et des comportements à risque en 1995, 1998 et 2000 chez les HSH recrutés dans différents sites de Barcelone ; de décrire les tendances sur la connaissance et la perception des thérapies anti-rétrovirales depuis 1998, et d’étudier les liens entre la pratique des relations anales non protégées et la perception des traitements anti-rétroviraux. Méthodes Quatre études transversales (1,2) ont été menées depuis 1993 avec la participation d’une association gaie (Stop Sida). Un échantillon représentatif de HSH a été recruté chaque année dans trois saunas, deux sex-shops, un lieu de drague dans un parc public, et également dans le fichier d’adresse d’une association gaie. Tous ces lieux de rendezvous étaient situés dans la ville de Barcelone. Utilisé depuis 1995, le questionnaire a été validé et adapté à partir de celui réalisé par l’Institut Universitaire de Médecine Sociale et Préventive, Lausanne (13). Il recueille les informations relatives aux données démographiques, aux réseaux sociaux, aux pratiques sexuelles avec les partenaires stables et occasionnels, à l’utilisation de drogues, aux comportements vis à vis des tests de dépistage du VIH, et à la connaissance et perception des traitements anti-rétroviraux et, depuis 1998, de la prophylaxie post-exposition. Les comportements décrits se rapportent aux 12 mois précédant l’étude. ➤ C The objectives of this paper are to describe the trends in prevalence of HIV infection and in risk behaviour in 1995, 1998, and 2000 among MSMs recruited in different venues in Barcelona; to describe trends in knowledge and perception of antiretroviral treatment since 1998; and to examine relationships between the practice of unprotected anal intercourse and the perception of antiretroviral treatment. Methods Four cross-sectional surveys (1,2) have been carried out since 1993, with the participation of a community-based gay organisation (Stop sida). A convenient sample of MSMs was recruited each year in three saunas, two sex shops, a pick up site in a public park and through a mailing list of a community-based gay organisation. All venues were located in Barcelona. The questionnaire, used since 1995, was validated and adapted from one developed by the Institut Universitaire de Médecine Sociale et Préventive, Lausanne (13). It collects information about demographic data, social network, sexual practices with steady and casual partners, drug use, HIV testing behaviour, and, since 1998, knowledge and perception of antiretroviral treatment and post-exposure prophylaxis. Les questionnaires ont été renvoyés par courrier. Pour évaluer la prévalence de l’infection à VIH, près de 300 prélèvements salivaires ont été demandés chaque année afin de déterminer la présence d’anticorps anti-VIH utilisant les techniques standard immunoenzymatiques (14). De plus, entre 1000 et 1400 questionnaires ont également été envoyés à tous les membres masculins d’une association gaie en Catalogne. Une analyse statistique descriptive a été réalisée en utilisant le test de 2 pour comparer les proportions et la régression logistique pour ajuster les estimations. Le logiciel statistique SPSS-PC V10 a été utilisé. Lorsque les différences n’étaient pas statistiquement significatives entre les trois études, seuls les résultats pour l’année 2000 ont été pris en compte. Les relations anales non protégées avec le partenaire stable et occasionnel ont été calculées comme la proportion de ceux qui n’ont jamais, ou occasionnellement, utilisé de préservatifs les 12 mois précédant l’enquête pendant des rapports sexuels avec des partenaires stables ou occasionnels. Les réponses sur la connaissance et la perception des traitements anti-rétroviraux (TAR) avaient cinq niveaux : fortement d’accord (1), d’accord (2), pas d’accord (3), pas du tout d’accord (4), ne sait pas (5). Pour l’analyse, certaines options ont été regroupées (1+2 = d’accord), (3+4 = pas d’accord), et 5 (ne sait pas) a été supprimée. Résultats Le taux de réponse du questionnaire variait de 29% (741/2543) en 1995 à 27% (713/2775) en 1998 et 24% (828/3445) en 2000. L’âge moyen a augmenté au fil des années (respectivement 33,7, 34,8 et 36,1 ans en 1995, 1998 et 2000, p<0,001). Hormis l’âge, les personnes sélectionnées présentaient les mêmes caractéristiques socio-démographiques. Le niveau d’éducation était élevé dans la plupart des cas, 49% étaient diplômées de l’enseignement universitaire et 70% vivaient à Barcelone. La grande majorité des homosexuels de l’étude avaient été dépistés pour la présence d’anticorps anti-VIH auparavant, et le nombre n’a fait qu’augmenter tout au long des années de l’étude (tableau 1). Nous avons également noté une augmentation du taux de ceux dépistés plus d’une fois (respectivement 40,9%, 50,9%, et 53,4% en 1995, 1998, et 2000). La prévalence auto-déclarée du VIH s’élevait à 16,4% [intervalle de confiance à 95%: 13,1-19,8] en 1995; 16,6% [13,4-20.5] en 1998; et 18,8% [15,7-21,9] en 2000. La prévalence du VIH estimée dans les prélèvements salivaires était de 14,2% [intervalle de confiance à 95%: 10,3-18,1] en 1995, 15,5% [11,3-19,8] en 1998, et 17,9% [13,622,2] en 2000. Bien qu’à la fois l’auto-déclaration et les études de prévalence VIH sur prélèvement salivaire montrent un accroissement, aucune n’est statistiquement significative. La plupart des répondants avaient eu des rapports sexuels avec des partenaires stables et occasionnels (53%), plus d’un tiers avec des partenaires occasionnels uniquement (36%), et 11% avec des partenaires stables seulement. Près d’un tiers avaient des rapports sexuels plusieurs fois par semaine, et 14% de manière irrégulière mais avec des périodes intensives. Bien que la fréquence de l’activité sexuelle paraisse stable, le nombre de partenaires sexuels montre une tendance à la hausse. En 2000, 58% avaient plus de 10 partenaires sexuels l’année précédente (respectivement 45% et 54% en 1995 et 1998, p<0,001). L’association reste significative après ajustement par l’âge (tableau 1). La pratique de relations sexuelles anales s’élevait à 87% avec un partenaire stable et à 83% avec un partenaire occasionnel, et les rapports oro-génitaux représentaient respectivement 98% et 96%. Les relations anales non protégées avec un partenaire stable ont été rapportées par 59% des répondants, et par 25% pour les partenaires ➤ Around 2000 anonymous self-administered questionnaires were distributed each year by four health educators of a gay association. Questionnaires were returned by mail. In addition, to estimate the prevalence of HIV infection, around 300 saliva samples were requested each year to determine HIV antibodies using standard enzymatic immunoassay techniques (14). Furthermore, between 1000 and 1400 questionnaires were also sent to all male members of a gay association in Catalonia. A descriptive statistical analysis was performed using 2 for comparisons of proportions and logistic regression to adjust estimates. The SPSS-PC V10 statistical software package was used. When differences were not statistically significant between the three surveys, only the results from the 2000 survey are presented. Unprotected anal intercourse (UAI) with a steady partner and with casual partners was calculated as the proportion of those who never or occasionally used a condom during the previous 12 months when practising anal intercourse with steady or casual partners. Statements about knowledge and perception of antiretroviral treatment (ARV) had five response options: strongly agree (1), agree (2), disagree (3), strongly disagree (4), do not know (5). For analysis, options have been collapsed (1+2 = agree; 3+4 = disagree) and option 5 (do not know) has been removed. Results The questionnaire return rate ranged from 29% (741/2543) in 1995 to 27% (713/2775) in 1998, and 24% (828/3445) in 2000. The mean age increased over the years (33.7, 34.8, and 36.1 years old in 1995, 1998, and 2000, respectively, p<0.001). With the exception of age, the selected samples were similar for demographic characteristics. Most of them had a high level of education (49% were university graduates) and were living in Barcelona (70%). The vast majority of the homosexual men studied had been tested for HIV antibodies previously, and this proportion increased over the years surveyed (table 1). We also observed an increase in the proportion of those who had been tested more than once (40.9%, 50.9%, and 53.4% in 1995, 1998, and 2000, respectively). Self-reported prevalence of HIV was 16.4% [95% confidence interval: 13.1-19.8] in 1995; 16.6% [13.4-20.5] in 1998; and 18.8% [15.7-21.9] in 2000. Prevalence of HIV estimated in the saliva samples was 14.2% [95% confidence interval: 10.318.1] in 1995, 15.5% [11.3-19.8] in 1998, and 17.9% [13.6-22.2] in 2000. Although both self-reported and saliva prevalence of HIV show an increasing trend, neither are statistically significant. Most respondents had sexual relationships with both steady and casual partners (53%), more than a third with casual partners only (36%), and 11% with steady partners only. Nearly a third had sexual encounters several times per week, and 14% irregularly, but with intensive periods. Although the frequency of sexual activity seems to be stable, the number of sexual partners shows an increasing trend. In 2000, 58% had more than 10 sexual partners during the previous year (45% and 54%, respectively, in 1995 and 1998, p<0.001). The association remains significant after adjusting for age (table 1). Anal intercourse was practised by 87% with a steady partner and 83% with casual partners, and oro-genital sex by 98% and 96%, respectively. Unprotected anal intercourse with a steady partner was reported by 59%, and with casual partners by 25% (table 1). In both cases the proportion of UAI showed a slight increasing trend, but was not statistically significant. Au cours des douze derniers mois, dans trente et un pour cent des cas, le préservatif s’était rompu, dans 22% des cas, il avait glissé. Les participants qui utilisaient des lubrifiants pendant les relations anales avaient moins d’accidents avec les préservatifs que ceux qui n’en utilisaient pas. Pourtant, moins de la moitié des hommes utilisant des préservatifs pendant les relations anales utilisaient toujours des lubrifiants ➤ During the previous 12 months, 31% of the participants had experienced at least one condom breakage and 22% condom slippage. The participants who always used lubricants during anal intercourse had fewer accidents with condoms than those who did not. However, less than half of the men who used condoms during anal intercourse always used lubricants (40%) and of these, 18% chose liposoluble lubricants (oil, petroleum jelly, cream, etc) that can reduce the elasticity of the condom and thus increase the risk of breakage. La prévalence de l’utilisation de drogues avant ou pendant les rapports sexuels les 12 mois précédant l’étude était élevée. A l’exception de l’alcool, l’utilisation de drogues révèle une tendance à la hausse au cours des années et reste significative après ajustement par l’âge (tableau 1). Responses to statements about ARV are presented in table 2. Although participants in 2000 seemed to be better informed, 5.6% still thought that “ARV therapies can cure HIV infection”. More respondents than in the previous survey reported “side effects of ARV therapies are very uncomfortable” (70.2% and 83.5% in 1998 and 2000, respectively). In 2000, more MSMs had heard about post-exposure prophylaxis (PEP) than in 1998: 21.5% believed that “with ARV it is possible to avoid HIV infection after a potential sexual risk exposure” versus 7.4% in 1998. En 2000, plus de HSH étaient au courant de la prophylaxie post-exposition (PPE) qu’en 1998 : 21,5% croyaient « qu’avec les TAR, il était possible d’éviter l’infection à VIH suite à une exposition sexuelle à risque potentiel » contre 7,4% en 1998. Globalement, en 2000, la perception des TAR était plus optimiste : une proportion plus élevée qu’en 1998 croyait « qu’avec les TAR, les HSH avaient moins peur d’être positifs pour le there was a more optimistic perception regarding ARV therapy : a higher proportion than in 1998 believed that “with ARV, MSMs are less afraid of becoming HIV positive”, and “pay less attention to prevention”, and are “less worried if they take sexual risks”. We found a significant association between the statements that suggest optimistic perception of ARV therapy and unsafe sex, and this remained significant after adjusting for age, year of survey, and known serological status (table 3). Nous avons trouvé une association significative entre les réponses qui suggèrent une perception optimiste des TAR et un comportement sexuel à risque. De même, ceux qui déclarent « avoir moins de crainte de devenir séropositif », « faire moins attention à la prévention »ou « être moins soucieux lors de comportements sexuels à risque » sont plus à même d’avoir des relations anales non protégées (odds ratio respectifs 1,9, 6,2, et 5,5). Commentaire Globalement, les résultats de cette quatrième étude n’ont pas révélé de changements significatifs dans la prévalence du VIH et les comportements sexuels à risque chez les HSH de l’étude à Barcelone. Seuls trois indicateurs ont montré une augmentation statistiquement significative : une proportion plus élevée de participants par rapport aux études précédentes ont déclaré avoir eu des rapports sexuels avec plus de 10 partenaires au cours des 12 mois précédant l’enquête ; ➤ transmit HIV” are 1.9 times more likely to have UAI with casual partners, and those who agree that “with ARV it is likely to avoid HIV infection after a potential sexual risk exposure“ are 1.7 times more likely. Similarly, those who report “being less afraid of becoming HIV positive”, “paying less attention to prevention” or “being less worried if they take sexual risks” are also more likely to have UAI (odds ratio 1.9, 6.2, and 5.5, respectively). Comment In general, the results of this fourth survey did not show significant changes in the prevalence of HIV and in sexual risk behaviour of MSMs surveyed in Barcelona. Only three indicators showed a statistically significant increase: a higher proportion of participants than previous surveys reported having had more than 10 male sexual partners in the 12 months before the survey; a higher proportion of ejaculation in the mouth with casual partners; and a higher prevalence of drug use during or before sexual encounters. This is not consistent with the recently reported increase in both the HIV incidence rate (3-5) and in HIV risk behaviour (9,10) among MSMs from other countries. Although cross-sectional studies suffer from methodological limitations, their repeated use over time with a standardised approach provides a useful way to describe the evolution of the HIV epidemic among MSMs. The return rate of questionnaires was not high, but was similar in the three surveys, and higher than in ➤ EUROSURVEILLANCE VOL. 7 - N° 2 FÉVRIER - FEBRUARY 2002 27 un taux plus élevé d’éjaculation dans la bouche avec des partenaires occasionnels ; et une prévalence plus élevée de l’utilisation de drogues pendant ou avant les rapports sexuels. Ceci n’est pas compatible avec l’augmentation récemment déclarée du taux d’incidence du VIH (3-5) et des comportements à risque (9,10) chez les HSH d’autres pays. Malgré les limites méthodologiques des études transversales, le fait de les poursuivre avec une approche standardisée permet de décrire utilement l’évolution de l’épidémie de VIH chez les HSH. Le taux de réponse des questionnaires n’était pas élevé bien qu’identique pour les trois études, et plus élevé que dans d’autres enquêtes (13). Nous ne pouvons pourtant pas généraliser les résultats de cette étude à tous les HSH de Barcelone. Les comportements sexuels auto-déclarés peuvent également être l’objet d’erreurs, mais nous avons essayé de les limiter en utilisant un questionnaire auto-administré anonyme. Les résultats de cette quatrième étude montrent cependant une prévalence toujours élevée du VIH chez les HSH à Barcelone, un niveau élevé de rapports sexuels non protégés avec des partenaires occasionnels et stables, et une prévalence élevée de l’utilisation de drogues pendant ou avant les rapports sexuels. De plus, parmi ceux qui utilisent des préservatifs, les accidents pendant leur manipulation restent importants. Malgré les campagnes d’information, l’usage de lubrifiants liposolubles est répandu. Nous avons également noté un optimisme croissant vis à vis des traitements anti-VIH, une plus grande insouciance au regard du VIH en raison des prophylaxies post-exposition, et une association significative entre l’optimisme mentionné et les relations anales non protégées avec les partenaires occasionnels, comme rapporté dans les autres études (11,12). other studies (13). We cannot, however, generalise findings from this study to all MSMs in Barcelona. Self-reported sexual behaviour may also be subject to measurement error, but we attempt to reduce this by using an anonymous self-completed questionnaire. The results of this fourth survey, however, still show a high prevalence of HIV among MSMs in Barcelona, a high level of unprotected sex with both steady and casual partners, and a high prevalence of drug use during or before sexual intercourse. In addition, among those who used condoms, accidents in the handling of condoms continued to be considerable and despite educational campaigns, the use of liposoluble lubricants is high. We also observed an increased trend of optimism regarding the efficacy of HIV combination treatment, a decreased worry about HIV infection because of PEP, and a significant association between optimism and UAI with casual partners, as reported in other studies (11,12). We conclude that there is no clear evidence of an increase in sexual risk behaviour among MSMs, but the optimism regarding ARV may lead to a greater relaxation in protective measures in the near future. In addition, the high prevalence of HIV and risk behaviour, the difficulties to maintain safer sex practices (“AIDS burnout”), the longer survival of people infected with HIV, and the ineffectiveness of treatments due to drug resistant HIV strains may make the control of the epidemic difficult. Further and more creative interventions that take into account the new scenario are needed. ■ Nous avons conclu que rien ne peut prouver l’augmentation des comportements sexuels à risque chez les HSH. On peut s’attendre à un plus grand relâchement des mesures de protection suite à l’optimisme dû aux traitements anti-rétroviraux. De plus, la prévalence élevée du VIH et des comportements à risque, les difficultés à maintenir des pratiques de safer sex (« burnout face au Sida »), la plus grande longévité des personnes infectées et l’inefficacité des traitements due aux souches résistantes pourraient bien limiter le contrôle de l’épidémie.
