REPORTING A CHANGE IN YOUR CIRCUMSTANCES
Marriage, birth, moving, divorce and any such changes in your circumstances may
affect your health cover and have to be notified to you sickness fund.
IF YOU GET MARRIED
If one of the spouses has no health coverage, he or she can be registered and covered
under the health cover of the insured spouse.
To be covered by the health insurance of your spouse, send following documents to your
sickness fund:

Form S3182A (attestation de la qualité d’ayant droit), available from your sickness fund
or from Internet at www.ameli.fr
A copy of your family book (livret de famille) or certificate of marriage.
IF YOU SETTLE WITH A PARTNER (OUTSIDE OF MARRIAGE)
If one of the partners has no health coverage, he or she can be registered and covered
under the health cover of the insured partner.
To be covered by the health insurance of your partner, send following documents to your
sickness fund:

or from Internet at www.ameli.fr or an affidavit stating that you and your partner are
living together
A copy of the dependent partner’s identity card or residence card (if the dependent is
not a national of a Member State of the European Economic Area) (1)
IF YOU ENTER A PACS (Civil Solidarity Pact)
If one of the partners has no health coverage, he or she can be registered and covered
under the health cover of the insured partner.
To be covered by the health insurance of your partner, send following documents to your
sickness fund:

BIRTH OF A CHILD
If both parents are insured, the child is covered under the health cover of either one or of
both parents.
If the whole family is covered under the health cover of one member, the child is registered
with his insured parent.
Send following documents to your sickness fund:

or from Internet at www.ameli.fr
IF YOU HAVE DIVORCED
If you are covered by your former partner’s health cover as a dependent*, entitlement
continues during 4 years after divorce has been ordered.
If you have or have had at least three dependent children, entitlement continues on a
permanent basis.
To be covered by the health insurance of your spouse, you have to send to your sickness
fund:

A copy of the divorce order or a certificate of birth mentioning that you are divorced
A copy of your family book (livret de famille) if you have dependent children
Your bank particulars (relevé d’identité bancaire) if you want the refunds to be
tranferred on a bank or postal account.
Upon termination of this four year period, if you have no coverage, you are entitled to the
Universal Health Coverage (Couverture Maladie Universelle, CMU). Get in touch with you
local sickness fund or with the social workers of you place of residence.
Children can be registered under the health insurance account of both parents. In certain
circumstances, this enables easier coverage (shared custody, holidays…)
IF YOUR ARE MOVING
Ø If you are moving within the same department
If you are moving within the same department notify your sickness fund of you new address.
If you have to register with a new local sickness fund, your former sickness fund with give
you the address of the new one.
Upon transfer of your file, you will receive a new entitlement certificate. The only thing that
you have to do is update your carte Vitale in a carte Vitale terminal.

Ø If you are moving to another department
Send to your sickness fund:

Form S1104 (« déclaration de changement de situation ») available from your sickness
fund or from Internet at www.ameli.fr or an affidavit stating that you and your partner
are living together
Your bank particulars (relevé d’identité bancaire) if you want the refunds to be
tranferred on a bank or postal account.
A copy of your family book (livret de famille) if you have dependent children
Your carte Vitale or, if you have lost it, a declaration of loss.
IF YOU CHANGE BANK
Send to your sickness fund:

Your new bank particulars (Relevé d’identité bancaire)
Write your Social Security number on all the papers that you send to your sickness fund.
Change of circumstances: do not forget to update your carte Vitale

Dependent (ayant droit): a person who is eligible to the refund of medical expenses on the
same basis as the insured (dependents include spouse, unmarried partner, PACS partner,
child or any other dependent person who has been living with the insured for at least one
year).
